摘要
目的探究经脐单孔腹腔镜手术对阑尾炎患儿术后机体微炎症指标及应激反应的影响。方法选取80例阑尾炎患儿,按随机数字表法分为两组,各40例。对照组采用常规三孔腹腔镜阑尾切除术(LA)治疗,观察组采取经脐单孔LA治疗。比较两组手术相关指标、微炎症指标[C反应蛋白(CRP)、白介素-6(IL-6)、肿瘤坏死因子-ɑ(TNF-ɑ)]、应激反应[去甲肾上腺素(NE)、皮质醇(Cor)]。结果两组手术时间比较,差异无统计学意义(P>0.05);观察组出血量较对照组少,肛门排气时间较对照组短(P<0.05)。术后1d,两组CRP、TNF-ɑ、IL-6较术前降低,观察组较对照组低(P<0.05)。术后1d,两组Cor、NE较术前高,但观察组较对照组低(P<0.05)。结论相较常规三孔LA,经脐单孔LA可减少阑尾炎患儿出血量,缩短肛门排气时间,减轻患儿微炎症反应及应激反应。
Objective To explore the effects of transumbilical single-port laparoscopic surgery on microinflammatory indexes and stress response in children with appendicitis after surgery.Methods A total of 80 children with appendicitis were selected and divided into two groups with 40 cases in each group according to random number table method.The control group was treated with conventional three-hole laparoscopic appendectomy(LA),and the observation group was treated with transumbilical single-hole LA.Surgery-related indicators,microinflammatory indicators[C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-alpha)],and stress responses[norepinephrine(NE),cortisol(Cor)]were compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05).The blood loss in the observation group was less and the anal exhaust time was shorter than that in the control group(P<0.05).1 day after surgery,CRP,TNF-alpha and IL-6 in both groups were reduced compared with those before surgery,and the observation group was lower than the control group(P<0.05).One day after surgery,Cor and NE were higher in both groups,but lower in observation group than in control group(P<0.05).Conclusion Compared with conventional three-hole LA,transumbilical single-hole LA can reduce the amount of blood loss in children with appendicitis,shorten the time of anal exhaust,and reduce the microinflammatory reaction and stress response.
