摘要
[目的]总结张杰教授使用防风通圣散的经验,探讨本方临证应用的思路与方法。[方法]选取张杰教授使用防风通圣散治疗银屑病、结节性痒疹、接触性皮炎、荨麻疹的验案四则,并结合历代名医名家对本方的相关论述,从医理病机、治则治法、药物加减特色等方面对验案进行分析。[结果]张杰教授认为,皮肤病虽病位在肌表,然形诸外者,往往病诸内,其病因常在内外合邪,“表里交攻、血分瘀热、风热湿毒”是其常见病机,治疗上强调解表清里、疏风清热、凉血利湿,用防风通圣散加减最为合拍。文中所举四则医案虽病名各异,病因不离“风、热、湿、瘀”,证型总属“表里俱盛”,证同治亦同,故均以防风通圣散加减治疗,对证处理,卓有疗效。[结论]张杰教授辨证论治与辨病用药相结合,以防风通圣散改汤加减治疗多种皮肤病,丰富了防风通圣散的使用范围,见解独到、特色鲜明,值得临床学习参考。
[Objective] To summarize Professor ZHANG Jie‘s experience in using Fangfeng Tongsheng Decoction, and explore the thinking and method of clinical application. [Methods] Four effective medical records of Professor ZHANG‘s use of Fangfeng Tongsheng Decoction in the treatment of psoriasis, pruritus nodosum, contact dermatitis and urticaria were selected and analyzed from the pathogenesis, treatment, the characteristics of drug addition and subtraction, combined with the discussion of the famous doctors of the past dynasties. [Results] Professor ZHANG believes that although skin diseases are located in the skin, however, the outward symptoms are due to the internal lesions, the etiology is often caused by internal and external evil. The common pathogenesis of skin diseases is “internal and external attack, blood division and heat stasis, wind heat and humidity toxicity”, the treatment emphasizes relieving the surface and clearing the interior, dredging wind and clearing heat, cooling blood and promoting dampness, and the addition or reduction of Fangfeng Tongsheng decoction is the most appropriate. Although the four medical cases mentioned in this paper had different names, the etiology was “wind, heat, dampness and blood stasis”, and the pathogenesis was always strong inside and outside. “The syndrome was the same as the treatment”, so all used the adding or subtracting of prescription for treatment according to the syndrome, the effect was significant. [Conclusion] Professor ZHANG combines the syndrome differentiation and disease differentiation, uses Fangfeng Tongsheng Decoction to treat many skin diseases, enriches the scope of use of this prescription, which has unique insight, worthy of referenceg learning.
