摘要
目的探讨对糖尿病合并高血压患者采用利拉鲁肽联合二甲双胍治疗的效果及对颈动脉斑块的影响。方法选择2019年1月—2021年12月昆山市第六人民医院收治的80例糖尿病合并高血压患者,随机分为采用二甲双胍治疗的对照组(n=40)与联合利拉鲁肽治疗的观察组(n=40),比较两组治疗前后血糖以及血压水平,检测治疗前后颈内动脉内膜中层厚度(IMT)与斑块大小、厚度、数量评估颈动脉斑块改变。结果治疗后,观察组空腹血糖(6.24±0.55)mmol/L、餐后2 h血糖(9.30±1.25)mmol/L、糖化血红蛋白(6.46±0.53)%,均低于对照组,差异有统计学意义(t=8.172、3.595、4.915,P<0.05)。治疗后,观察组收缩压低于对照组,IMT、斑块大小、斑块厚度、斑块数量、均小于对照组,差异有统计学意义(P<0.05)。结论对糖尿病合并高血压患者采用利拉鲁肽联合二甲双胍治疗疗效确切,能有效改善患者血糖与血压水平,能改善颈动脉斑块状态。
Objective To investigate the effect of treatment with liraglutide combined with metformin on carotid plaque in patients with diabetes mellitus combined with hypertension.Methods A total of 80 patients with diabetes mellitus and hypertension admitted to the Sixth People's Hospital of Kunshan from January 2019 to December 2021 were randomly divided into the control group treated with metformin(n=40)and the observation group treated with liraglutide(n=40).The blood glucose and blood pressure levels of the two groups were compared before and after treatment.The carotid artery intima-media thickness(IMT)and plaque size,thickness and number were measured before and after treatment to evaluate the changes of carotid plaque.Results After treatment,the fasting blood glucose(6.24±0.55)mmol/L,2 h postprandial blood glucose(9.30±1.25)mmol/L and glycosylated hemoglobin(6.46±0.53)%in the observation group were lower than those in the control group,and the differences were statistically significant(t=8.172,3.595,4.915,P<0.05).After treatment,the systolic blood pressure of the observation group was lower than that of the control group,IMT,plaque size,plaque thickness,plaque number,were less than the control group,the difference was statistically significant(P<0.05).Conclusion Treatment with liraglutide combined with metformin in patients with diabetes mellitus combined with hypertension is effective and can improve patients'blood glucose and blood pressure levels,and can improve carotid plaque status.
