妊娠早期空腹血糖在妊娠期糖尿病中的诊断价值 被引量:2

Diagnostic value of fasting plasma glucose to gestational diabetes mellitus in early pregnancy

摘要 目的评估妊娠早期空腹血糖(FPG)在预测妊娠期糖尿病(GDM)发病风险中的预测价值。方法本研究为2019年1月—2022年6月在郑州大学第三附属医院进行的病例对照研究。共有3566名孕妇纳入本研究,收集临床研究对象临床特征,以FPG水平进行分组,运用多因素logistic回归预测模型以及多个回归方程评估妊娠早期FPG与GDM发病风险的关系。结果妊娠早期FPG异常组孕妇(5.1 mmol/L≤FPG<7.0 mmol/L)GDM发生率高于FPG正常组(FPG<5.1 mmol/L)(P<0.001);纳入妊娠早期FPG、年龄、孕周、生化指标等多因素logistic回归预测模型为:logit(GDM)=-12.96715+1.48638×FPG+0.09582×年龄+0.02963×孕周+0.02183×ALT-0.03141×AST+0.01770×GGT+0.00474×UA。用多个回归方程评估妊娠早期FPG与GDM发生的风险,在校正了混杂因素后,与FPG对照组(FPG<4.44 mmol/L)相比,FPG水平异常组(5.1 mmol/L≤FPG<7.0 mmol/L)GDM发病风险升高(OR值分别为5.01、12.78、20.97)。结论妊娠早期FPG异常是GDM的高危因素,尤其是妊娠早期FPG≥5.1 mmol/L时,GDM发生率明显上升,妊娠早期FPG≥5.6 mmol/L时,则患GDM的风险成倍增长。 Objective This paper aims to investigate the value of fasting plasma glucose(FPG)levels during early pregnancy in the prediction of gestational diabetes mellitus(GDM).Methods This study was a cross-sectional study conducted in the Third Affiliated Hospital of Zhengzhou University from January 2019 to June 2022.A total of 3566 pregnant women were enrolled in the study.Multivariate logistic regression model and multiple regression equations were used to evaluate the relation⁃ship between FPG in early pregnancy and the risk of GDM.Results The incidence of GDM was high among women with an ab⁃normal FPG(5.1 mmol/L≤FPG<7.0 mmol/L)in early pregnancy than that among women with a normal FPG(FPG<5.1 mmol/L)(P<0.001).The multivariate logistic regression model was logit(GDM)=-12.96715+1.48638×FPG+0.09582×age+0.02963×week of gestation+0.02183×ALT-0.03141×AST+0.01770×GGT+0.00474×UA.Mul⁃tiple regression equations were used to assess the risk of FPG and GDM in early pregnancy.After adjusting for confounder fac⁃tors,compared with FPG control group(FPG<4.44 mmol/L),the risk of GDM in the group with abnormal FPG level(5.1 mmol/L≤FPG<7.0 mmol/L)was increased(OR=5.01,OR=12.78,OR=20.97).Conclusion Abnormal FPG in early pregnancy is a high risk factor for GDM,especially with FPG≥5.1 mmol/L in early pregnancy,and the incidence of GDM increases;with FPG≥5.6 mmol/L in early pregnancy,the risk of GDM increases exponentially.
