摘要
目的分析超声乳化白内障吸除术联合人工晶状体植入术治疗晶状体悬韧带异常继发性闭角型青光眼的效果。方法84例晶状体悬韧带异常继发性闭角型青光眼患者,随机分为对照组和观察组,各42例。两组均给予超声乳化白内障吸除术,对照组联合小梁切除术治疗,观察组联合人工晶状体植入术治疗。比较两组手术前后的视力改善情况、眼压、前房深度,并发症发生情况。结果术前,观察组视力(0.08±0.03)与对照组的(0.08±0.02)比较,差异无统计学意义(t=0,P=1>0.05)。术后,观察组视力(0.61±0.16)高于对照组的(0.53±0.14),差异有统计学意义(P<0.05)。术后,观察组眼压(14.68±6.46)mm Hg(1 mm Hg=0.133 kPa)低于对照组的(18.35±6.25)mm Hg,前房深度(2.32±0.49)mm高于对照组的(2.00±0.32)mm,差异有统计学意义(P<0.05)。观察组并发症发生率4.76%低于对照组的19.05%,差异有统计学意义(P<0.05)。结论给予晶状体悬韧带异常继发性闭角型青光眼患者超声乳化白内障吸除术联合人工晶状体植入术治疗能够显著提升临床疗效和安全性,具有推广价值。
Objective To analyze the effect of phacoemulsification combined with intraocular lens implantation in the treatment of angle-closure glaucoma secondary to abnormal lens suspensory ligament.Methods A total of 84 patients with angle-closure glaucoma secondary to abnormal lens suspensory ligament were randomly divided into control group and observation group,with 42 cases in each group.Both groups were treated with phacoemulsification,on this basis,the control group was treated with trabeculectomy,and the observation group was treated with intraocular lens implantation.The improvement of visual acuity,intraocular pressure and anterior chamber depth before and after surgery,and complications were compared between the two groups.Results Before surgery,there was no statistically significant difference in visual acuity between the observation group[(0.08±0.03)]and the control group[(0.08±0.02)](t=0,P=1>0.05).After surgery,the visual acuity of(0.61±0.16)in the observation group was higher than that of(0.53±0.14)in the control group,and the difference was statistically significant(P<0.05).After surgery,the intraocular pressure of(14.68±6.46)mm Hg(1 mm Hg=0.133 kPa)in the observation group was lower than that of(18.35±6.25)mm Hg in the control group;and the anterior chamber depth of(2.32±0.49)mm in the observation group was higher than that of(2.00±0.32)mm in the control group;the differences were statistically significant(P<0.05).The complication rate of 4.76%in the observation group was lower than that of 19.05%in the control group,and the difference was statistically significant(P<0.05).Conclusion Phacoemulsification combined with intraocular lens implantation in the treatment of angle-closure glaucoma secondary to abnormal lens suspensory ligament can significantly improve the clinical efficacy and safety,which has the value of promotion.
