摘要
目的:探讨术中风险管理对妇科腹腔镜手术患者凝血指标、深静脉血栓(DVT)、胃肠功能的影响。方法:选取2019年4月—2021年9月厦门市湖里区妇幼保健院收治的108例妇科腹腔镜手术患者作为研究对象。根据随机数表法将其分为对照组和观察组,各54例。对照组给予术中常规护理,观察组给予术中风险管理。比较两组干预前、干预后2 d凝血指标,DVT发生率及胃肠功能。结果:干预后2 d,两组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)均缩短,观察组长于对照组,纤维蛋白原(FIB)水平升高,观察组低于对照组,差异有统计学意义(P<0.05)。观察组DVT发生率低于对照组,差异有统计学意义(P<0.05)。两组肠鸣音恢复时间、肛门排气时间、肛门排便时间比较,差异无统计学意义(P>0.05)。结论:术中风险管理可以改善妇科腹腔镜手术患者术后的凝血指标,减少DVT的发生。
Objective:To explore the effect of intraoperative risk management on coagulation index,deep vein thrombosis(DVT)and gastrointestinal function in patients undergoing gynecological laparoscopic surgery.Method:A total of 108 patients undergoing gynecological laparoscopic surgery admitted to Xiamen Huli District Maternal and Child Care Hospital from April 2019 to September 2021 were selected as the study objects.They were divided into control group and observation group according to random number table method,with 54 cases in each group.The control group was given intraoperative routine nursing,and the observation group was given intraoperative risk management.Coagulation index before intervention and 2 d after intervention,incidence of DVT and gastrointestinal function were compared between the two groups.Result:After 2 d of intervention,the prothrombin time(PT),activated partial thrombin time(APTT)and thrombin time(TT)in both groups were shortened,but the observation group was longer than that in the control group,and fibrinogen(FIB)levels were increased,the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).The incidence of DVT in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).There was no significant difference in bowel sound recovery time,anal exhaust time and anal defecation time between the two groups(P>0.05).Conclusion:Intraoperative risk management can improve the postoperative coagulation index in patients undergoing gynecological laparoscopic surgery,and reduce the occurrence of DVT.
