摘要
目的 探讨加味温胆汤对反流性食管炎患者胃肠动力及肠道菌群的影响。方法 选取2018年3月—2020年9月北京中医药大学房山医院收治的150例反流性食管炎患者的病例资料进行分析,按治疗方式分为对照组和观察组各75例。对照组采取西医常规疗法,观察组在对照组基础上加用加味温胆汤治疗。比较两组患者治疗疗效,反流性疾病诊断问卷(RDQ)评分,中医证候积分,胃肠动力指标[胃泌素(GAS)、促胃动素(MTL)]水平,炎症因子水平以及肠道菌群水平。结果 治疗后,观察组临床有效率为94.67%,高于对照组患者76.00%(P<0.05)。两组患者治疗后RDQ评分及中医证候总积分均降低,且观察组低于对照组(P<0.05)。两组患者治疗后血清GAS、MTL水平均高于治疗前,观察组水平更高(P<0.05)。两组患者治疗后炎症因子水平均降低,对照组高于观察组(P<0.05)。两组患者治疗后肠杆菌、肠球菌数量低于治疗前,对照组高于观察组(P<0.05);两组患者治疗后乳酸杆菌、双歧杆菌菌群数量高于治疗前,观察组高于对照组(P<0.05)。结论 温胆汤用于反流性食管炎具有较强的疗效,能够改善患者胃肠动力,降低患者炎症反应,可增加患者肠道有益菌数量,促进肠道菌群平衡。
Objective To investigate the effect of Wendan Decoction(温胆汤)on gastrointestinal motility and intestinal flora in patients with reflux esophagitis.Methods The case data of 150 patients with reflux esophagitis admitted to our hospital from March 2018 to September 2020 were retrospectively analyzed,and they were divided into control group and observation group according to treatment methods,with 75 cases in each group.The control group was treated with conventional Western medicine,and the observation group was treated with Wendan Decoction on the basis of the control group.The therapeutic effect,gastrointestinal motility,inflammatory factors and intestinal flora levels were compared between the two groups.Results After treatment,the clinical effective rate of the observation group was 94.67%,which was higher than the control group 76.00%(P<0.05).RDQ score and total score of TCM syndrome in the two groups were decreased after treatment,and the observation group was lower than the control group(P<0.05).After treatment serum GAS,MTL level of the two groups were higher than before the treatment,observation group was higher(P<0.05).The levels of inflammatory factors in the two groups were decreased after treatment,the control group was higher than the observation group(P<0.05).After treatment,the numbers of Enterobacter and Enterococcus of the two groups were lower than before the treatment,control group was higher than the observation group(P<0.05).After treatment,the numbers of Lactobacillus and Bifidobacterium were increased in both groups,and the observation group was higher than the control group(P<0.05).Conclusion Wendan Decoction has strong curative effect in patients with reflux esophagitis,which can improve gastrointestinal motility,reduce inflammation,increase the number of intestinal beneficial bacteria and promote the balance of intestinal flora.
