摘要
目的探讨腹膜后节细胞神经瘤(GN)CT影像特征及肿瘤包绕大血管、累及胸腔情况。资料与方法回顾性分析2016年2月—2021年10月在北京大学国际医院行CT检查和手术治疗的39例腹膜后GN,评估肿瘤大小、形态、生长方式、是否累及胸腔、数量、密度、增强表现及与附近大血管的关系。结果肿瘤最大径(11.10±4.32)cm;形态不规则26例,规则13例;沿组织间隙呈铸型生长37例;经膈肌主动脉裂孔钻入胸腔6例;单发肿块21例,多发结节18例;肿瘤平扫密度较低且均匀,动脉期、静脉期肿瘤轻度强化,延迟扫描渐进性强化;肿瘤包绕大血管25例。结论复杂性腹膜后GN除平扫密度较低且均匀、沿组织间隙呈铸型生长、延迟扫描渐进性强化等CT特征外,还有肿块较大,多发,包绕大血管和钻入胸腔等特征。对肿瘤是否包绕大血管和钻入胸腔准确评估,能够为手术提供有价值的信息。
Purpose To investigate the preoperative assessment and diagnosis of CT imaging features in patients with complicated retroperitoneal ganglioneuroma(GN),and to evaluate the involvement of thoracic cavity and adjacent great blood vessels.Materials and Methods A total of 39 patients with retroperitoneal GN underwent CT scanning and operation in Peking University International Hospital from February 2016 to October 2021 were retrospectively included.The imaging features of GN were analyzed,including size,contour,growth patterns,extension of thoracic cavity,numbers,density,enhanced patterns and relationships with peripheral blood vessels.Results The maximum diameter of lesions was(11.10±4.32)cm.There were 26 cases with regular shape and 13 cases with irregular shape.There were 37 cases with growth of drilling seam and 6 cases with extending into thoracic cavity through aortic fissure in the diaphragm.There were 21 cases with single focus and 18 cases with multiple foci.GN lesions showed homogeneous low density on plain CT images,slightly enhanced density on artery phase and venous phase,and continuously enhanced density on delay phase.There were 25 cases with enclosing the adjacent great blood vessels.Conclusion In addition to homogeneous low density on plain CT images,and continuously enhancement patterns on delay phase,the CT imaging features of complicated retroperitoneal GN show larger tumor size,multiple foci,the growth of drilling seam and enclosing great blood vessels,and extending into thoracic cavity.The involvement of thoracic cavity and adjacent great blood vessels are accurately evaluated via ultrasound to provide useful information for operation.
