摘要
目的:探究人体成分分析在妊娠期糖尿病(Gestational Diabetes Mellitus,GDM)患者个体化体重管理以及合理膳食中的应用。方法:选取2021年3月~2022年3月期间中山火炬开发区人民医院产检诊断为GDM的92例孕妇作为研究对象,所有患者按照随机数字表法分为对照组和观察组,各46例。对照组采取常规治疗和常规GDM饮食的干预方案,观察组在对照组干预方案的基础上联合人体成分分析结果进行个体化体重管理及合理膳食。观察两组孕妇体重增长情况、血糖水平及妊娠结局。结果:观察组产前体重、体重增长幅度相比对照组均明显降低(P<0.05)。干预后,两组孕妇空腹血糖、餐后2 h血糖、糖化血红蛋白均有明显下降(P<0.05);且观察组均明显低于对照组(P<0.05)。在羊水过多、早产发生率上,观察组明显低于对照组(P<0.05)。在新生儿低血糖、巨大儿发生率上,观察组明显低于对照组(P<0.05)。结论:基于人体成分分析的个体化体重管理以及合理膳食指导,有利于控制GDM患者血糖水平,维持体重合理增长,改善妊娠及新生儿结局。
Objective:To explore the application of body composition analysis in individualized weight management and reasonable diet of patients with gestational diabetes(GDM).Methods:From March 2021 to March 2022,92 pregnant women diagnosed as GDM by obstetric examination in the People's Hospital of Zhongshan Torch Development Zone were selected and included in the study.They were divided into two groups according to the method of random number table.The control group included 46 pregnant women who took routine treatment and routine GDM diet.The observation group included 46 pregnant women who took body composition analysis on the basis of the above,and combined with the results to carry out individualized weight management and reasonable diet.The weight gain,blood glucose level and pregnancy outcome of the two groups were observed.Results:Compared with the control group,the prenatal weight and the growth range of body weight in the observation group were significantly reduced,and the difference was statistically significant(P<0.05).After intervention,the fasting blood glucose,2-hour postprandial blood glucose and glycosylated hemoglobin of pregnant women in the two groups decreased compared with those in the same group before intervention,with a statistically significant difference(P<0.05);The observation group was significantly lower than the control group,the difference was statistically significant(P<0.05).The incidence of polyhydramnios and premature delivery in the observation group was significantly lower than that in the control group(P<0.05).The incidence of neonatal hypoglycemia and macrosomia in the observation group was significantly lower than that in the control group,with a statistically significant difference(P<0.05).Conclusion:Individualized weight management based on body composition analysis and reasonable dietary guidance are helpful to control the blood glucose level of GDM patients,maintain reasonable weight growth,and improve pregnancy and neonatal outcomes.
