多模态影像技术诊断肉芽肿性乳腺炎的应用价值 被引量:2

The value of mastitis by multimodal imaging in diagnosis of granulomatrus mastitis

摘要 目的探讨彩色多普勒超声和MRI及两者联合诊断肉芽肿性乳腺炎的应用价值。方法选取在我院完成手术或穿刺活检的641例患者,均被病理证实为肉芽肿性乳腺炎,对比超声、MRI及二者联合应用诊断肉芽肿性乳腺炎的准确率。结果在诊断肉芽肿乳腺炎的641例患者中,彩色多普勒超声诊断的准确率为86.9%,MRI诊断准确率为85.5%,超声和MRI诊断准确率比较,差异无统计学意义(P>0.05),两者联合诊断准确率96.4%,彩色多普勒超声和MRI联合应用诊断准确率明显高于超声和MRI诊断准确率(P<0.05)。超声特征:大多表现为形态不规则、边界不清的非均质肿块。MRI特征,病灶在T_(1)WI呈低信号,T_(2)WI抑脂序列呈高信号,DWI呈不均匀高信号,对应ADC呈低信号,增强扫描不均匀性强化并内部分隔渐进性强化,TIC曲线呈III型314例、II型168例、I型15例,并乳晕区皮肤增厚。结论彩色多普勒超声及MRI能较高显示肉芽肿性乳腺炎内部结构和血流动力学特征,两者可以相互补充。两者联合应用能提高诊断准确率。 Objective To explore the value of color Doppler ultrasonography,magnetic resonance imaging(MRI)and the combined of both in diagnosis of granulomatous mastitis.Methods We collected the data during the period of complete surgery or biopsy,641 patients were confirmed by pathology granulomatous mastitis.Ultrasound image data and MRI image data were collected.We analyzed ultrasound,MRI,and the combined use of both in diagnosis of granulomatous mastitis,granulomatous mastitis and analyzed clinical and ultrasound and MRI imaging features.Results In the diagnosis of granulomatous mastitis of 641 cases,color doppler ultrasound diagnostic accuracy was about 86.9%,the MRI diagnostic accuracy was about 85.5%,ultrasound and MRI diagnostic accuracy had no statistical significance(P>0.05),the joint diagnosis accuracy rate was 96.4%,the color doppler ultrasonography and magnetic resonance imaging(MRI)combined diagnostic accuracy was significantly higher than ultrasound and MRI diagnostic accuracy(P<0.05).Ultrasonography was characterized by heterogeneous masses with irregular morphology and unclear boundaries.MRI features were as follows:lesions showed low signal in T_(1)WI,high signal in T_(2)WI lipid inhibition sequence,uneven high signal in DWI,low signal in ADC,uneven enhancement in enhanced scan and progressive enhancement in internal space.TIC curve showed 314 cases of type III,168 cases of type II,and 15 cases of type I,and skin thickening in areola area.Conclusion Color Doppler ultrasound and MRI can better show the internal structure and hemodynamic characteristics of granulomatous mastitis,which can complement each other.Combined application of the two methods can improve the accuracy of diagnosis of granulomatous mastitis and has higher application value in clinical diagnosis and treatment.
