摘要
目的:观察DP化疗方案联合三维适形调强放疗用于食管癌患者的效果。方法:回顾性分析2019年10月至2021年12月该院收治的96例食管癌患者的临床资料,按照治疗方式不同将其分为对照组和研究组各48例。对照组实施三维适形调强放疗,研究组在对照组基础上联合DP化疗方案治疗,比较两组客观缓解率、治疗前后血清学指标[血管内皮生长因子(VEGF)、胸苷激酶1(TK1)、表皮生长因子受体(EGFR)]水平、肿瘤标志物[鳞状上皮细胞癌抗原(SCCA)、癌胚抗原(CEA)、糖类抗原19-9(CA19-9)]水平和生命质量[欧洲癌症患者生命质量测定量表(EORTC QLQ-C30)]评分。结果:两组客观缓解率比较,差异无统计学意义(P>0.05);治疗后,两组VEGF、TK1、EGFR、CA19-9、CEA、SCCA水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后,两组EORTC QLQ-C30评分均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:DP化疗方案联合三维适形调强放疗用于食管癌患者可提高生命质量评分,降低肿瘤标志物和血清学指标水平,效果优于单纯三维适形调强放疗。
Objective:To observe effects of DP chemotherapy combined with three-dimensional conformal intensity modulated radiotherapy in treatment of patients with esophageal cancer.Methods:The clinical data of 96 patients with esophageal cancer admitted to the hospital from October 2019 to December 2021 were retrospectively analyzed.They were divided into control group and study group according to different treatment methods,48 cases in each.The control group was treated with three-dimensional conformal intensity-modulated radiotherapy,while the study group was treated with DP chemotherapy on the basis of that of the control group.The objective remission rate,the serological index levels[vascular endothelial growth factor(VEGF),thymidine kinase1(TK1),epidermal growth factor receptor(EGFR)],the tumor marker levels[squamous cell carcinoma antigen(SCCA),carcinoembryonic antigen(CEA),carbohydrate antigen 19-9(CA19-9)],and the quality of life score[European quality of life scale for cancer patients(EORTC QLQ-C30)]were compared between the two groups before and after the treatment.Results:There was no significant difference in the objective remission rate between the two groups(P>0.05).After the treatment,the levels of VEGF,TK1,EGFR,CA19-9,CEA and SCCA in the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,after the treatment,the EORTC QLQ-C30 scores of the two groups were higher than those before the treatment,that in the study group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusions:DP chemotherapy combined with three-dimensional conformal intensity-modulated radiotherapy for the patients with esophageal cancer can improve the quality of life scores and reduce the levels of tumor markers and serological indexes.Moreover,it is superior to single three-dimensional conformal intensity modulated radiotherapy.
