摘要
目的观察心肺康复运动训练在慢性阻塞性肺疾病(COPD)稳定期患者中的应用效果。方法120例COPD稳定期患者,以随机数字表法分为对照组和研究组,每组60例。对照组予以常规康复指导,研究组予以心肺康复运动训练。比较两组患者干预前后的肺功能指标(呼吸频率、血氧饱和度、静脉血氧分压、最大通气量、6 min步行距离)水平、Borg呼吸困难指数评分以及肺功能康复效果、2个月内急性发作情况。结果干预后,两组患者呼吸频率、Borg呼吸困难指数评分均低于干预前,血氧饱和度、静脉血氧分压、最大通气量、6 min步行距离均高于对照组,且研究组患者呼吸频率(19.31±1.27)次/min、Borg呼吸困难指数评分(1.41±0.37)分低于对照组的(22.16±1.86)次/min、(2.17±0.45)分,血氧饱和度(98.92±0.38)%、静脉血氧分压(92.51±2.48)mm Hg(1 mm Hg=0.133 kPa)、最大通气量(54.58±5.73)L/min、6 min步行距离(429.27±51.01)m均高于对照组的(96.51±0.71)%、(90.42±2.06)mm Hg、(51.42±4.91)L/min、(398.54±47.11)m,差异均具有统计学意义(P<0.05)。研究组患者肺功能康复总有效率100.00%高于对照组的90.00%,2个月内急性发作率1.67%低于对照组的13.33%,差异具有统计学意义(P<0.05)。结论给予COPD稳定期患者心肺康复运动训练指导,可显著改善患者肺功能指标,减轻呼吸疲劳程度,获得理想的康复效果,并有效的减少干预后的急性发作率,效果理想。
Objective To analyze the effect of cardiopulmonary rehabilitation exercise training in patients with stable chronic obstructive pulmonary disease(COPD).Methods A total of 120 patients with stable COPD were divided into control group and research group according to random numerical table,with 60 cases in each group.The control group was given routine rehabilitation guidance,and the research group was given cardiopulmonary rehabilitation exercise training.Both groups were compared in terms of levels of lung function indexes(respiratory rate,blood oxygen saturation,renous partial pressure of oxygen,maximum voluntary volume,6-min walking distance)and Borg dyspnea index score before and after intervention,pulmonary function rehabilitation effect and acute attacks within 2 months.Results After intervention,the respiratory rate and Borg dyspnea index score in both groups were lower than those before intervention in this group;the blood oxygen saturation,partial pressure of oxygen,maximum voluntary volume and 6-minute walking distance in both groups were higher than those before intervention in this group;the respiratory rate of(19.31±1.27)times/min and Borg dyspnea index score of(1.41±0.37)points in the research group were lower than those of(22.16±1.86)times/min and(2.17±0.45)points in the control group;in the research group,the blood oxygen saturation was(98.92±0.38)%,the renous partial pressure of oxygen was(92.51±2.48)mm Hg(1 mm Hg=0.133 kPa),the maximum voluntary volume was(54.58±5.73)L/min,and the 6-min walking distance was(429.27±51.01)m,which were higher than those of(96.51±0.71)%,(90.42±2.06)mm Hg,(51.42±4.91)L/min,and(398.54±47.11)m in the control group;all the differences were statistically significant(P<0.05).The total effective rate of pulmonary function rehabilitation was 100.00%in the research group,which was higher than that of 90.00%in the control group;the acute attack rate within 2 months was 3.23%in the research group,which lower than that of 13.33%in the control group;the differences were statistically significant(P<0.05).Conclusion Cardiopulmonary rehabilitation exercise guidance has ideal effect for patients with stable chronic obstructive pulmonary disease(COPD),which can significantly improve the lung function index of patients,reduce the degree of respiratory fatigue,achieve ideal rehabilitation effect,and effectively reduce the frequency of acute attack after intervention.
