不同心功能分级慢性心力衰竭患者超声心动图参数及TLR-4信号通路蛋白水平观察 被引量:4

Observation of echocardiographic parameters and TLR-4 signal pathway protein levels in patients with chronic heart failure with different concentric functional grades

摘要 目的 观察不同心功能分级慢性心力衰竭患者超声心动图参数及Toll样受体4(TLR-4)信号通路蛋白水平变化。方法 回顾性选取2021年4月至2022年4月大庆市第三医院收治的84例慢性心力衰竭患者作为观察组,按心功能分级将其分为Ⅰ级21例,Ⅱ级22例,Ⅲ级20例,Ⅳ级21例,另选同期30名健康体检者作为对照组。比较不同心功能分级组与对照组间超声心电图参数及TLR-4信号通路蛋白水平,包括左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)、左心室射血分数(LVEF)、每分输出量(CO)、每搏输出量(SV)、心脏指数(CI)、TLR-4、核因子-κB(NF-κB)、髓样分化因子(MyD88)蛋白的表达情况;Spearman相关性分析超声心动图参数与TLR-4信号通路蛋白水平与心功能分级的相关性。结果 观察组LVESD、LVEDD、LVESV、TLR-4、NF-κB、MyD88水平均明显高于对照组,LVEF明显低于对照组,差异均有统计学意义(P<0.05),但两组LVEDV、CO、SV、CI水平相比,差异均无统计学意义(P>0.05);LVESD、LVEDD、LVESV、TLR-4、NF-κB、MyD88水平随心功能等级递增而逐渐增加,LVEF随心功能等级递增而逐渐减小,差异均有统计学意义(P<0.05),但各心功能等级中LVEDV、CO、SV、CI水平相比,差异无统计学意义(P>0.05)。进行Spearman检验得知,LVESD、LVEDD、LVESV、TLR-4、NF-κB、MyD88与心功能等级呈显著正相关(P<0.05),LVEF与心功能等级呈显著负正相关(P<0.05)。结论 LVESD、LVEDD、LVESV、TLR-4、NF-κB、MyD88与心功能等级呈显著正相关性,LVEF与心功能等级呈显著负正相关性,说明不同心功能分级患者,存在明显的超声心动图参数变化与TLR-4信号通路蛋白水平异常。 Objective To observe the changes of echocardiographic parameters and Toll like receptor 4(TLR-4)signal pathway protein levels in patients with chronic heart failure with different concentric functional grades.Methods A total of 84 patients with chronic heart failure admitted to Daqing Third Hospital from April 2021 to April 2022 were retrospectively selected as the observation group,which were divided into 21 patients of grade I,22 patients of grade II,20 patients of grade III,and 21 patients of grade IV according to the cardiac function classification.Another 30 healthy people in the same period were selected as the control group,and the echocardiogram parameters and TLR-4 signal pathway protein levels,including the left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),left ventricular end systolic volume(LVESV),left ventricular end diastolic volume(LVEDV),left ventricular ejection fraction(LVEF),minute output(CO),stroke volume(SV),and cardiac index(CI),TLR-4,nuclear factor-κB(NF-κB),MyD88 protein expression;Spearman correlation analysis was used to analyzed the correlation between echocardiographic parameters,TLR-4 signal pathway protein levels and cardiac function grading.Results The levels of LVESD,LVEDD,LVESV,TLR-4,NF-κB and MyD88 in the observation group were significantly higher than those in the control group,and LVEF was significantly lower than that in the control group,the differences were statistically significant(P<0.05),but there was no statistically significant difference in the levels of LVEDV,CO,SV and CI between the two groups(P>0.05).The levels of LVESD,LVEDD,LVESV,TLR-4,NF-κB and MyD88 were increased and gradually increased,while the levels of LVEF were increased and gradually decreased,the differences were statistically significant(P<0.05),but there was no statistically significant difference in the levels of LVEDV,CO,SV and CI among all cardiac function levels(P>0.05).Spearman test showed that LVESD,LVEDD,LVESV,TLR-4,NF-κB and MyD88 were significantly positively correlated with cardiac function grade,while LVEF was significantly negatively correlated with cardiac function grade(P<0.05).Conclusion LVESD,LVEDD,LVESV,TLR-4,NF-κB and MyD88 were significantly positively correlated with cardiac function grade,while LVEF was significantly negatively correlated with cardiac function grade,indicating that there were obvious changes in echocardiogram parameters and abnormal levels of TLR-4 signaling pathway protein in patients with different cardiac function grades.
