基于ICF理念的核心肌锻炼在股骨头坏死关节置换术后患者康复护理中的应用价值

Application Value of Core Muscle Exercise Based on the Concept of ICF in Patients with Necrosis of Femoral Head and Joint Replacement

摘要 目的探讨基于国际功能、残疾和健康分类(ICF)理念的核心肌锻炼在股骨头坏死(NFH)关节置换术后患者中的应用价值。方法回顾性选取河南省直第三人民医院2020年6月至2022年7月收治的121例接受全髋关节置换术(THR)的NFH患者作为研究对象,依照康复护理干预方式分为康复组和训练组。康复组(60例)接受阶段化康复护理,训练组(61例)接受基于ICF理念的核心肌锻炼,对比两组髋关节功能、步行能力、血清肌酸激酶水平及不良反应发生情况。结果干预3个月后两组关节畸形与活动度、行走能力、疼痛程度、生活能力评分较干预前升高,且训练组高于康复组(P<0.05)。训练组干预3个月后步长、步速、6 min最大步行距离、步频水平大于康复组(P<0.05)。重复测量方差分析显示,血清肌酸激酶水平的主效应、交互效应、时间效应有统计学意义(P<0.05);两组血清肌酸激酶水平呈逐渐降低趋势,训练组低于康复组(P<0.05)。训练组不良反应发生率[3.28%(2/61)]低于康复组[8.33%(5/60)],差异无统计学意义(P>0.05)。结论基于ICF理念的核心肌锻炼可改善接受THR术的NFH患者的血清肌酸激酶水平,调节髋关节功能,调控步行能力,有良好安全性。 Objective To explore the application value of core muscle exercise based on the international classification of functioning,disability and health(ICF)in elderly patients after joint replacement with necrosis of femoral head(NFH).Methods A total of 121 patients with NFH who underwent total hip replacement(THR)in the Third People’s Hospital of Henan Province from June 2020 to July 2022 were retrospectively selected as research objects,and divided into rehabilitation group and training group according to rehabilitation intervention methods.The rehabilitation group(60 cases)were given staged rehabilitation nursing,and the training group(61 cases)were given core muscle exercise based on ICF.Hip function,walking ability,serum creatine kinase level and the occurrence of adverse reactions were compared between the two groups.Results After 3 months’intervention,the scores of joint deformity,range of motion,walking ability,pain degree and living ability in two groups were higher than those before intervention,and the training group were higher than the rehabilitation group(P<0.05).After 3 months’intervention,the walking length,walking speed,6 minutes maximum walking distance and step frequency of the training group were higher than those of the rehabilitation group(P<0.05).Repeated measures analysis of variance showed that the main effect,interaction effect and time effect of serum creatine kinase level were statistical different(P<0.05).The serum creatine kinase level in both groups was gradually decreased,and the training group was lower than the rehabilitation group(P<0.05).The incidence of adverse reactions in the training group[3.28%(2/61)]was lower than that[8.33%(5/60)]in the rehabilitation group,the difference was not statistically significant(P>0.05).Conclusion The core muscle exercise based on ICF concept can improve serum creatine kinase level of patients with NFH undergoing THR,regulate hip function and regulate walking ability,with good safety.
