摘要
目的:探讨结核分枝杆菌抗体(TB-Ab)、γ-干扰素释放试验(IGRA)、GeneXpert技术联合检测在肺结核合并糖尿病诊断中的效能。方法:回顾性分析2019年1月至2022年12月于惠安县医院就诊的200例疑似肺结核合并糖尿病患者的临床资料,均行TB-Ab、IGRA、GeneXpert技术检测,以痰培养结果为金标准,分析三者单项及联合检测诊断肺结核的效能。结果:痰培养结果显示,200例疑似肺结核合并糖尿病患者中,177例肺结核,占88.50%(177/200),23例非肺结核,占11.50%(23/200);TB-Ab检测检出阳性186例,占93.00%(186/200);IGRA检测检出阳性187例,占93.50%(187/200);GeneXpert技术检出阳性162例,占81.00%(162/200);联合检测检出阳性175例,占87.50%(175/200);GeneXpert技术诊断肺结核的特异度、阳性预测值均高于TB-Ab、IGRA单项检测,灵敏度低于TB-Ab、IGRA单项检测,差异均有统计学意义(P<0.05);联合检测诊断肺结核的准确度、阴性预测值均高于TB-Ab、IGRA、GeneXpert技术单项检测,差异均有统计学意义(P<0.05)。结论:TB-Ab、IGRA、GeneXpert技术联合检测诊断肺结核的效能高于三者单项检测。
Objective:To investigate efficacy of Mycobacterium tuberculosis antibody(TB-Ab)detection,interferon-γ release assay(IGRA)combined with GeneXpert technology in diagnosis of pulmonary tuberculosis complicated with diabetes.Methods:The clinical data of 200 patients with suspected pulmonary tuberculosis complicated with diabetes who were treated in the hospital from January 2019 to December 2022 were retrospectively analyzed.TB-Ab,IGRA and GeneXpert were detected in all patients.Using the results of sputum culture as the gold standard,the efficacy of combined detection of the three technologies in the diagnosis of pulmonary tuberculosis was analyzed.Results:The results of sputum culture showed that among the 200 patients with suspected pulmonary tuberculosis complicated with diabetes,177 cases were diagnosed as pulmonary tuberculosis,accounting for 88.50%(177/200),and 23 were non-pulmonary tuberculosis cases,accounting for 11.50%(23/200).TB-Ab test detected 186 positive cases,accounting for 93.00%(186/200).IGRA detected 187 positive cases,accounting for 93.50%(187/200).GeneXpert technology detected 162 positive cases,accounting for 81.00%(162/200).The combined detection detected 175 positive cases,accounting for 87.50%(175/200).The specificity and the positive predictive value of GeneXpert technology in the diagnosis of pulmonary tuberculosis were higher than those of single TB-Ab detection and single IGRA detection,the sensitivity was lower than that of single TB-Ab detection and single IGRA detection,and the differences were statistically significant(P<0.05).The accuracy and the negative predictive value of the combined detection in the diagnosis of pulmonary tuberculosis were higher than those of single TB-Ab detection,single IGRA detection and single GeneXpert single detection,and the differences were statistically significant(P<0.05).Conclusions:The efficacy of combined detection of TB-Ab,IGRA and GeneXpert in the diagnosis of pulmonary tuberculosis complicated with diabetes is higher than those of three single detections.
