摘要
目的 分析24 h血糖浓度联合尿蛋白检测在慢性糖尿病患者早期肾脏损伤诊断中的应用价值。方法选取2021年6月—2023年1月新沂市人民医院收治的185例疑似慢性糖尿病早期肾损伤患者为研究对象,以肾小球滤过率(glomerular filtration rate, GFR)检测结果为金标准。分析24 h血糖浓度联合尿蛋白检验慢性糖尿病早期肾损伤患者阳性率、特异度、灵敏度。结果 在185例疑似慢性糖尿病早期肾损伤的病例中,GFR诊断阳性患者183例(98.92%),阴性2例(1.08%)。24 h血糖浓度联合尿蛋白检测诊断与金标准(GFR诊断结果)具有一致性,该诊断的灵敏度为98.36%,特异度为100.00%。结论 24 h血糖浓度联合尿蛋白这一检测方式在早期肾损伤诊断中的应用与金标准检出结果相接近,可见该检验方式具有一定应用价值,可较为准确地检出患者早期肾损伤发生的情况,为医生制订诊疗方案提供参考依据。
Objective To analyze the value of 24 h blood glucose concentration combined with urine protein detection in the diagnosis of early renal injury in patients with chronic diabetes mellitus.Methods A total of 185 suspected chronic diabetic patients with early renal injury treated in Xinyi People´s Hospital from June 2021 to January 2023 were selected as the study objects,and the glomerular filtration rate(GFR)detection result was used as the gold stan‐dard.The positive rate,specificity and sensitivity of 24 h blood glucose concentration combined with urine protein test were analyzed in patients with early renal injury of chronic diabetes.Results Of the 185 suspected cases of early renal injury with chronic diabetes,183 cases(98.92%)and 2 cases(1.08%)were GFR positive.The 24 h blood glucose con‐centration combined with urine protein detection was consistent to the gold standard(GFR diagnosis result),with a sensitivity of 98.36%and a specificity of 100.00%.Conclusion The application of the detection method of 24 h blood glucose concentration combined with urinary protein in the diagnosis of early kidney injury is close to the gold stan‐dard detection result,indicating that this test method has certain application value,and can more accurately detect the occurrence of early kidney injury in patients,providing a reference for doctors to make diagnosis and treatment plans.
