摘要
目的:研究胃癌幸存者社会疏离现状和影响因素,为临床医护干预提供依据。方法:采用便利抽样法选取2017年1月至2022年12月医院治疗的胃癌幸存者186例,采用一般疏离量表调查其社会疏离现状,通过单因素分析和多重线性回归分析胃癌幸存者社会疏离影响因素。结果:胃癌幸存者社会疏离评分为(45.39±6.55)分,抑郁自评量表(SDS)评分为(56.33±8.34)分,社会支持评分为(31.23±3.27)分;SDS评分与社会疏离总分及其4个维度评分均呈正相关,社会支持与社会疏离总分及其4个维度评分均呈负相关,SDS评分与社会支持总分及其3个维度评分呈负相关(P<0.05)。单因素和多重线性回归分析显示:临床分期、治愈后时间段、家庭人均月收入、放化疗情况、SDS评分、社会支持评分均为胃癌患者社会疏离独立影响因素(P<0.05),性别不为其影响因素(P>0.05);临床分期Ⅲ~Ⅳ期、放疗或化疗或放疗+化疗、SDS评分均为胃癌幸存者社会疏离风险因素(P<0.05);治愈后时间≥3年、家庭人均月收入≥5000元、社会支持评分为均患者社会疏离保护性因素(P<0.05)。结论:胃癌幸存者社会疏离水平较高,针对患者社会疏离影响因素,临床、家属、社会等应及时干预,提高患者融入家庭、回归社会的能力。
Objective:To study the status and influencing factors of social isolation among gastric cancer survivors,and to provide a reference for clinical medical intervention.Methods:A convenient sampling method was used to select gastric cancer survivors treated in hospital from January 2017 to December 2022.The social isolation status of patients was investigated by General Alienation Scale.The influencing factors of social isolation among gastric cancer survivors were analyzed through univariate analysis and multiple linear regression analysis.Results:The social isolation scores of gastric cancer survivors was(45.39±6.55),the Self Depression Scale(SDS)score was(56.33±8.34),and the social support score was(31.23±3.27).The SDS score was positively correlated with the total score of social isolation and the four dimensional scores,while social support was negatively correlated with the total score of social isolation and the four dimensional scores.SDS score was negatively correlated with the total score of social support and the three dimensional scores(P<0.05).Univariate analysis and multiple linear regression analysis showed that clinical stage,time after cure,family monthly income,radiation and chemotherapy status SDS score,and social support score were independent influencing factors for social isolation in gastric cancer patients(P<0.05),while gender was not the influencing factor(P>0.05).Clinical stageⅢtoⅣ,radiotherapy or chemotherapy or radiotherapy+chemotherapy,SDS scores were all risk factors for social isolation among gastric cancer survivors(P<0.05).Time after cure≥3 years,family monthly income≥5000 yuan and social support score were protective factors for social isolation among patients(P<0.05).Conclusions:The gastric cancer survivors have a high level of social isolation.According to the influencing factors of social isolation among patients,clinical,family members,and society should promptly intervene to improve the ability of integrating into families and returning to society.
