摘要
目的:探究不同触地习惯对跑步时足弓、第一跖趾关节六自由度(six degrees of freedom, 6DOF)运动以及足底筋膜长度的影响。方法:采用高速双平面荧光透视成像系统(100 Hz)和三维测力台(2 000 Hz)同步采集30名健康男性跑者[习惯前掌着地(forefoot strike,FFS)跑者和习惯后跟着地(rearfoot striker,RFS)跑者各15名]在跑步(跑速为3 m/s)支撑期右脚跟骨、第一跖骨和第一近节趾骨的荧光图像以及地面反作用力数据。通过CT图像建立3D骨骼模型和坐标系,并进行3D-2D配准,获取足弓、第一跖趾关节的6DOF数据以及足底筋膜长度。结果:相比RFS跑者,FFS跑者:1)跖趾关节趾屈肌力更大(P<0.05);2)足弓的内外、前后、屈伸和内收外展活动度显著更大(P<0.05);3)第一跖趾关节的前后、上下、屈伸和内收外展活动度显著更大(P<0.05);4)足底筋膜的最大应变、最大长度和功率也显著更大(P<0.05)。结论:习惯前掌着地跑者足弓和足底筋膜储存和释放的能量更多,跖趾关节的蹬伸效果以及足底筋膜的力学特性更强。此外,前掌着地跑者也具有更大的跖趾关节屈肌力量。
Objective:To explore effects of different foot strike patterns on the six degrees of freedom(6DOF)data of the arch,first metatarsophalangeal joint and plantar fascia length during running.Methods:30 healthy male runners were divided into two groups,15 forefoot strike(FFS)runners and 15 rearfoot strike(RFS)runners.The high-speed dual fluoroscopic imaging system(100 Hz)and 3D force platform(2000 Hz)were used to acquire fluoroscopic images of the calcaneus,first metatarsal,phalanges and ground reaction force data during the stance phase of running(3 m/s)simultaneously.The 3D bone model and coordinate system were established by the CT images,combined with 3D-2D registration to obtain the 6DOF data of arch,first metatarsophalangeal joint and plantar fascia length.Results:Compared with RFS runners,FFS runners:1)Toe flexor strength was increased significantly(P<0.05);2)the range of motion in medial-lateral,anterior-posterior,plantarflexion-dorsiflexion and internal and external rotation motions of arch was increased significantly(P<0.05);3)the range of motion for anterior-posterior,superior-inferior,plantarflexion-dorsiflexion and internal and external rotation motions of the first metatarsophalangeal joint was increased significantly(P<0.05);4)the maximum strain,maximum length and power of the plantar fascia increased significantly(P<0.05).Conclusions:Running with FFS may increase the energy storage and release of the arch and plantar fascia,as well as the push-off effect of the metatarsophalangeal joint.Meanwhile,FFS enhances the mechanical properties of the plantar fascia potentially.In addition,FFS runners have greater toe flexor strength.
