摘要
目的探讨完壁式与开放式乳突切开I期鼓室成形术的特点及术后疗效。方法回顾分析85例(耳)慢性化脓性中耳炎及中耳胆脂瘤患者的临床资料。其中25例患者行完壁式乳突切开(CWU)伴I期鼓室成形术;60例患者行开放式乳突切开(CWD)伴I期鼓室成形术。观察术中听小骨病变程度,观察术后干耳时间,术后外耳道及移植物恢复情况,有无并发症。术后3个月开始定期复查听力,以最后一次复查的听力结果作为统计分析资料。结果①术中观察指标:听小骨病变的程度。②术后观察指标:干耳时间、外耳道形态和功能、移植物愈合情况、术后听力、术后并发症及随访等。结论两种方法各有优势,完壁式较开放式鼓室成形术后听力改善更佳。最终手术方案仍需根据术中情况而定。
Objective To compare the feature and effect of canal wall up mastoidectomy and canal wall down mastoidectomy combined with one-stage tympanoplasty.Method The records of 85 cases(ears)treated for chronic suppurative otitis media(CSOM)and cholesteatoma were retrospectively reviewed.Canal wall up(CWU)mastoidectomy combined with one-stage tympanoplasty was done in another 25 cases,and canal wall down(CWD)mastoidectomy combined with one-stage tympanoplasty was done in 60 cases.The ossicles lesion during surgery,the dry-up time,recovery of external auditory canal,graft uptake,or any complications after surgery were observed.Audiologic follow-up began regularly 3 months after surgery,and the latest hearing outcomes were used for analysis.Result①Intraoperative observation indicators:the state of ossicles.②Postoperative observation indicators:dry-up time,appearance and function of external auditory canal,graft uptake,post-operative hearing performance and complications after surgery.Conclusion Both CWU and CWD had its own advantages,however CWU got a better hearing improvement than CWD.The final surgical plan still needs to be determined according to the intraoperative situation.
