摘要
目的了解新冠疫情下基于宁波市妇幼健康服务体系提供的孕产期保健服务工作现状,及其所采取的主要应对措施。方法采用分层目的抽样和方便抽样,从市妇幼保健院、区/县妇幼保健院以及社区卫生服务中心提供孕产期保健服务的医务人员中选取26名受访者。采用个人半结构式深度访谈,于2022年8-9月对受访者进行一对一访谈,在NVivo11软件辅助下,采用Colaizzi 7步分析法对资料进行分析。结果宁波市建立了完善的三级妇幼健康服务体系网络。疫情期间,针对产检地点,可为疑似病例或确诊病例提供医学隔离或新型冠状病毒肺炎定点医院;对有特殊需求的孕妇,可为其提供灵活的产检服务地点。针对产检次数及就诊及时性,孕妇产检次数略有减少,产检间隔视情况适当延长,但均能满足基本保障。针对服务方式,能充分利用信息化等手段,推进“云上妇幼”等服务,但仍需关注孕产妇心理健康问题。结论疫情期间,宁波市孕产期保健,在产检地点、产检次数以及服务方式方面均做出了相应调整,兼顾了基本服务及抗疫需求。本次总结的经验可为新冠以及其他类似传染病疫情下妇幼保健工作防控提供参考。
Objective To understand the current status and the responses of prenatal care based on maternal and child health service system in Ningbo during the COVID-19 epidemic.Methods Stratified sampling and convenience sampling methods were used to select 26 interviewees among the medical staff who provided prenatal care services from the municipal and county-level maternal and child health center,as well as community health service centers in Ningbo.Semi-structured in-depth interviews were conducted with interviewees from August to September in 2022.The Colaizzi 7-step method was used for qualitative data analysis by NVivo11 software.Results A three-tier maternal and child health system in Ningbo has been well established.During the COVID-19 epidemic,designated hospitals or quarantine wards were provided for pregnant women with suspected/confirmed novel coronavirus pneumonia for prenatal examination.The flexible prenatal care choices were provided for the pregnant woman with the special needs.Both of the two conditions were under the recommendation although the frequency of prenatal examinations decreased somehow and the interval of prenatal examinations extended appropriately.In terms of service mode,informatization technology was applied to promote service such as"cloud-based maternal and child health".Meanwhile,more attention about maternal mental health should be drawn in the future.Conclusions During the COVID-19 epidemic,prenatal care service in Ningbo had been adjusted accordingly in terms of the location,frequency,and mode of service delivery,which considered both basic service supply and the prevention and control of the epidemic.The lessons from this study can provide a reference for maternal and child health care services during the COVID-19 epidemic and other similar infectious diseases.
