摘要
目的观察保留自主神经的腹腔镜右半结肠癌根治术的可行性及临床应用价值。方法选取2020年6月—2022年7月收治的80例结肠癌患者,根据随机数字表分为两组,对照组和实验组各40例,两组患者均接受腹腔镜右半结肠癌根治术治疗,对照组术中进行神经部分切除,而实验组进行保留自主神经的根治术,比较两组手术相关指标、术后胃肠功能恢复指标、并发症发生率。结果实验组手术用时短于对照组,术中出血量少于对照组(P<0.05或0.01),实验组的首次排气时间、肠鸣音恢复时间较对照组短(P<0.05),两组淋巴结清扫数目、正常进食时间、住院时间比较差异均无统计学意义(P>0.05)。实验组的总并发症发生率低于对照组,差异有统计学意义(P<0.01)。结论保留自主神经的腹腔镜右半结肠癌根治术是可行的,可有效清扫淋巴结,预防术后胃肠功能紊乱。
Objective To observe the feasibility and clinical value of laparoscopic radical resection of right hemicolonic carcinoma with autonomic nerve preservation.Methods A total of 80 patients with colon cancer admitted to hospital from June 2020 to July 2022 were selected and divided into control group(n=40)and experimental group(n=40)according to random number table.All patients received laparoscopic radical resection of right colon cancer.The control group underwent partial resection of nerve,while the experimental group underwent radical resection of autonomic nerve preservation.And then,operation related indexes,postoperative gastrointestinal function recovery indexes and complication rates were compared between the two groups.Results The operation time of the experimental group was shorter than that of the control group,the amount of intraoperative blood loss was less than that of the control group,and the recovery time of the first exhaust and bowel sound of the experimental group was shorter than that of the control group(P<0.05 or 0.01).There were no statistically significant differences in the number of lymph nodes dissection,the time of normal eating and the length of hospital stay between the two groups(P>0.05).The total incidence of complications in the experimental group was lower than that in the control group,and difference was statistically significant(P<0.01).Conclusion Laparoscopic radical resection of right hemicolonic carcinoma with autonomic nerve preservation is feasible,which can effectively remove lymph nodes,and prevent postoperative gastrointestinal dysfunction.
