摘要
目的探讨环泊酚复合阿芬太尼用于患者无痛胃肠镜诊疗的有效性和安全性。方法选取2022年8月至10月长江大学附属第一医院收治的100例择期行无痛胃肠镜的患者作为研究对象,使用随机数字表法将患者分为环泊酚组(50例)与丙泊酚组(50例),两组各剔除1例患者,最终两组各49例。两组患者均给予4μg/kg阿芬太尼后,环泊酚组静脉给予环泊酚注射液0.5 mg/kg,丙泊酚组静脉给予丙泊酚中/长链脂肪乳注射液2 mg/kg。比较两组患者的镇静成功率、睫毛反射消失时间、药物追加次数、苏醒时间、生命体征变化、围手术期不良事件发生率。结果两组的镇静成功率比较,差异无统计学意义(P>0.05)。环泊酚组药物追加次数少于丙泊酚组,麻醉药物重复追加例数少于丙泊酚组,苏醒时间长于丙泊酚组,差异有统计学意义(P<0.05)。术中生命体征,环泊酚组在胃镜通过喉部时的脉搏血氧饱和度、呼吸频率均高于丙泊酚组,差异有统计学意义(P<0.05)。环泊酚组围手术期不良事件总发生率低于丙泊酚组,环泊酚组术中注射痛、低氧血症及梦境的发生率均少于丙泊酚组,差异有统计学意义(P<0.05)。结论环泊酚复合阿芬太尼应用于无痛胃肠镜的麻醉效果比丙泊酚复合阿芬太尼的药物追加次数少,呼吸循环更稳定,术后不良事件的发生率更少,应用于无痛内镜诊疗的麻醉安全有效。
Objective To investigate the efficacy and safety of Ciprofol combined with Alfentanil in painless gastrointestinal endoscope.Methods From August to October 2022,100 patients with painless gastrointestinal endoscope who were admitted to the First Affiliated Hospital of Yangtze University were selected as the study subjects.Patients were divided into Ciprofol group(50 cases)and Propofol group(50 cases)using random number table method.Each of the two groups 1 patient was excluded,and 49 patients were in each group.After 4μg/kg of Afentanil was administered to both groups,the Ciprofol group was given Ciprofol Injection 0.5 mg/kg,and the Propofol group was given Propofol Medium and Long Chain Fat Emulsion Injection 2 mg/kg.The success rate of sedation,disappearance time of eyelash reflex,times of drug addition,recovery time,changes of vital signs and perioperative adverse events were compared between the two groups.Results There was no significant difference in the success rate of sedation between the two groups(P>0.05).The number of drug addition times in Ciprofol group was less than that in Propofol group,the number of repeated anesthetic drug addition cases was less than that in Propofol group,and the recovery time was longer than that in Propofol group,the differences were statistically significant(P<0.05).Intraoperative vital signs,the pulse oxygen saturation and respiratory rate of Ciprofol group were higher than those of Propofol group during laryngoscopy,and the differences were statistically significant(P<0.05).The total incidence of perioperative adverse events in Ciprofol group was lower than that in Propofol group,and the incidence of intraoperative injection pain,hypoxemia and dream in Ciprofol group was lower than that in Propofol group,with statistical significance(P<0.05).Conclusions The anaesthesia effect of Ciprofol combined with Alfentanil in painless gastrointestinal endoscope is less drug additions than that of Propofol combined with Alfentanil,with more stable respiratory circulation and fewer postoperative adverse events.So Ciprofol combined with Alfentanil applied in painless gastrointestinal endoscope diagnosis and treatment is more safe and effective.
