^(99m)Tc-DTPA肾动态显像与过敏性紫癜性肾炎新月体形成率的相关性研究

摘要 目的探讨^(99m)锝-二乙基三胺五乙酸(^(99m)Tc-DTPA)肾动态显像指标与过敏性紫癜性肾炎(Henoch-Schönlein purpura nephritis,HSPN)新月体形成率之间的相关性。方法选取2013年4月至2020年10月嘉兴市第一医院收治的HSPN患者80例,按照新月体形成率将患者分成A组(新月体形成率<15%)、B组[新月体形成率15%~25%(含)]和C组(新月体形成率25%~50%)。所有患者均接受^(99m)Tc-DTPA肾动态显像检查,并计算灌注指数(perfusion index,PI)和肾小球滤过率(glomerular filtration rate,GFR),比较各组肾功能及肾图曲线,分析GFR、PI与新月体形成率间的关系。结果A组与B组、A组与C组间GFR及PI差异有统计学意义(P<0.05),B组与C组GFR及PI差异无统计学意义(P>0.05)。结论HSPN患者随着疾病进展和新月体形成比例升高,GFR逐渐下降,肾脏PI升高。^(99m)Tc-DTPA肾动态显像能无创、动态反映HSPN肾脏损伤的严重程度,对于早期发现肾功能损害及准确评价治疗效果和预后有非常重要的价值。 Objective To investigate the relationship between ^(99m)Tc-DTPA renal dynamic imaging parameters and crescent formation rate in Henoch-Schönlein purpura nephritis(HSPN).Methods A total of 80 HSPN patients admitted to the First Hospital of Jiaxing from April 2013 to October 2020 were included.According to the rate of crescentic formation,the patients were divided into group A(crescent formation rate<15%),group B(crescent formation rate 15%-25%),and group C(crescent formation rate 25%-50%).All patients underwent ^(99m)Tc-DTPA renal dynamic imaging,and their perfusion index(PI)and glomerular filtration rate(GFR)were calculated.Renal function and renogram curves were compared among 3 groups,and the relationship between GFR,PI,and crescent formation rate was analyzed.Results Statistical analysis showed that there was a statistically significant difference in GFR and PI between group A and B,and between group A and C(P<0.05),while there was no statistically significant difference in GFR and PI between group B and C(P>0.05).Conclusion As the disease progresses,the crescent formation rate and perfusion index in HSPN patients gradually increases,and the glomerular filtration rate gradually decreases.^(99m)Tc-DTPA renal dynamic imaging can noninvasive and dynamically reflect the severity of renal injury in HSPN,which is of great value for early detection of renal function damage,as well as accurate evaluation of treatment effectiveness and prognosis.
