摘要
目的 分析予以肾结石患者输尿管软镜碎石术(flexible ureteroscope lithotripsy, FURL)与经皮肾镜碎石术(percutaneous nephrolithotomy, PCNL)治疗的临床作用。方法 选择2020年4月-2023年4月在邳州市人民医院接受治疗的70例肾结石患者,依据随机数表法分为两组,接受PCNL治疗的35例为对照组,接受FURL治疗的35例为观察组。对比两组围术期指标、肾功能指标、应激指标、炎症因子指标及并发症情况。结果 两组手术时间、一期结石清除率对比,差异无统计学意义(P>0.05);观察组术中出血量,术后下床、住院时间分别为(13.25±2.45)mL、(21.56±6.37)h、(3.64±1.15)d,均优于对照组的(35.26±8.57)mL、(42.12±7.58)h、(6.23±1.57)d,差异有统计学意义(t=14.609、12.285、7.873,P<0.05)。术后24 h,观察组肾功能指标、应激指标及炎症因子指标均较对照组更低,差异有统计学意义(P<0.05)。观察组并发症发生率为5.71%低于对照组的22.86%,差异有统计学意义(χ^(2)=4.200,P<0.05)。结论 予以肾结石患者FURL、PCNL治疗均可发挥显著的结石清除作用,但FURL创伤更小、对肾功能的影响更小、应激反应及并发症更少,术后恢复更快。
Objective To analyze the clinical effects of flexible ureteroscope lithotripsy(FURL)and percutaneous nephrolithotripsy(PCNL)in the treatment of renal calculi.Methods Seventy cases of renal calculi patients treated in Pizhou People´s Hospital from April 2020 to April 2023 were selected.They were divided into two groups according to random number table method,35 cases treated with PCNL as the control group and 35 cases treated with FURL as the observation group.Perioperative indexes,renal function indexes,stress indexes,inflammatory factor indexes and com⁃plications were compared between the two groups.Results There was no statistically significant difference in the com⁃parison of operation time and phase I stone removal rate between the two groups(P>0.05).Intraoperative bleeding,postoperative bed and hospitalization time in the observation group were(13.25±2.45)mL,(21.56±6.37)h,(3.64±1.15)d,respectively,which were better than that in the control group of(35.26±8.57)mL,(42.12±7.58)h,and(6.23±1.57)d,and the difference was statistically significant(t=14.609,12.285,7.873,P<0.05).At 24 h postoperatively,renal function indexes,stress indexes and inflammatory factor indexes in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The complication rate of 5.71%in the observa⁃tion group was significantly lower than that in the control group 22.86%,and the difference was statistically significant(χ^(2)=4.200,P<0.05).Conclusion The treatment of FURL and PCNL to patients with renal calculi can play a signifi⁃cant role in stone removal,but FURL is less traumatizing,has less impact on renal function,has less stress and com⁃plications,and has a faster postoperative recovery.
