2型糖尿病患者血糖水平、Hp感染对糖尿病肾病发生率影响及与肾脏损伤程度的相关性分析

摘要 目的 探究2型糖尿病患者血糖水平、幽门螺杆菌(helicobacter pylori, Hp)感染与糖尿病肾病之间的关系。方法 回顾性分析柳州市柳铁中心医院2020年10月—2021年12月期间内分泌科收治及健康体检中检出的203例2型糖尿病患者临床资料,将其以是否存在糖尿病肾病分为无糖尿病肾病组(101例)、糖尿病肾病组(102例);将糖尿病肾病患者以尿白蛋白/肌酐比值(urinary albumin/creatinine ratio, UACR)水平将其分为微量蛋白尿组(50例)、临床蛋白尿组(52例),以14C-UBT表示Hp感染程度,分析其血糖水平、Hp与糖尿病肾病发生率及肾脏损伤程度之间的关系。结果 糖尿病肾病组空腹血糖(fasting plasma glucose, FPG)、糖化血红蛋白(glycosylated hemoglobin, HbA1c)水平及Hp感染发生率较无糖尿病肾病组高,差异有统计学意义(P<0.05);Logistic回归分析显示,HbA1c、Hp感染为糖尿病肾病独立危险因素(P<0.05);FPG与UACR无相关性(P>0.05);HbA1c水平、14C-UBT水平与UACR水平呈正相关(P<0.05)。结论 HbA1c水平及Hp感染为2型糖尿病患者糖尿病肾病发生独立危险因素,且HbA1c水平升高、14C-UBT水平与患者肾功能损伤程度有关。 Objective To explore the relationship between blood glucose level,helicobacter pylori(Hp)infection and diabetic nephropathy in patients with type 2 diabetes.Methods Retrospective analysis was made on the clinical data of 203 patients with type 2 diabetes who were admitted to the Endocrinology Department of Liuzhou Liutie Central Hospital from October 2020 to December 2021 and found in the health examination.They were divided into non diabetes nephropathy group(101 cases)and diabetes nephropathy group(102 cases)according to whether there was diabetes nephropathy.The patients with diabetes nephropathy were divided into micro proteinuria group(50 cases)and clinical proteinuria group(52 cases)according to the urinary albumin/creatinine ratio(UACR)level.The infection degree of Hp was expressed by 14C-UBT,and the relationship between blood glucose level,Hp and the incidence of diabetes nephropathy and the degree of kidney damage was analyzed.Results The fasting plasma glucose,glycated hemoglobin level and Hp infection rate in the diabetes nephropathy group were higher than those in the non diabetes nephropathy group,the difference was statistically significant(P<0.05).Logistic regression analysis showed that HbA1c and Hp infection were independent risk factors for diabetes nephropathy(P<0.05).There was no correlation between FPG and UACR(P>0.05).The levels of HbA1c and 14C-UBT were positively correlated with the levels of UACR(P<0.05).Conclusion HbA1c level and Hp infection are independent risk factors for diabetic nephropathy in patients with type 2 diabetes,and elevated HbA1c level and 14C-UBT level are related to the degree of renal function damage in patients.
