摘要
目的 研究甘精胰岛素+二甲双胍+结合雌激素软膏治疗2型糖尿病合并老年性阴道炎的效果。方法选取2020年7月-2022年7月福建省三明市清流县妇幼保健院收治的58例2型糖尿病合并老年性阴道炎患者为研究对象,根据治疗方法不同分为对照组(29例,二甲双胍+结合雌激素软膏治疗)和观察组(29例,甘精胰岛素+二甲双胍+结合雌激素软膏治疗),对比两组治疗效果、临床症状积分、血糖指标、阴道环境、炎症因子、不良反应、复发率。结果 观察组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组阴道瘙痒、阴道疼痛、阴道分泌物积分均低于对照组,差异有统计学意义(P<0.05)。观察组患者治疗后的空腹血糖、餐后2 h血糖、阴道pH值均低于对照组,差异有统计学意义(P<0.05)。观察组治疗后乳酸杆菌检出率高于对照组,差异有统计学意义(P<0.05)。观察组患者治疗后白细胞介素6、肿瘤细胞因子α、超敏C反应蛋白均低于对照组,差异有统计学意义(P<0.05)。观察组患者不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。观察组患者治疗后3、6个月的复发率均低于对照组,但差异无统计学意义(P>0.05)。结论 甘精胰岛素+二甲双胍+结合雌激素软膏治疗在2型糖尿病合并老年性阴道炎治疗中效果显著,可提高治疗效果,改善临床症状、血糖水平、阴道环境、炎症因子水平,且具有较高的安全性。
Objective To study the effect of insulin glargine+metformin+conjugated estrogen ointment in the treatment of type 2 diabetes mellitus complicated with senile vaginitis.Methods 58 patients with type 2 diabetes complicated with senile Vaginitis admitted to Qingliu County Maternal and Child Health Hospital,Sanming,Fujian Province from July 2020 to July 2022 were selected as the research objects.According to different treatment methods,they were divided into the control group(29 cases,treated with Metformin+combined estrogen ointment)and the observation group(29 cases,treated with Insulin glargine+Metformin+combined estrogen ointment),and the treatment effects,clinical symptom scores,blood glucose indicators Vaginal environment,inflammatory factors,adverse reactions,recurrence rate.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The scores of vaginal itching,vaginal pain and Vaginal discharge in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).After treatment,the Glucose test#Fasting blood sugar,blood glucose 2 hours after meal,and vaginal pH of the patients in the observation group were lower than those in the control group,the difference was statistically signifi‐cant(P<0.05).The detection rate of lactobacilli in the observation group after treatment was higher than that in the control group,the difference was statistically significant(P<0.05).Observation group patients treated with interleukin 6 and tumor cytokinesα,The levels of hypersensitive C-reactive protein were all lower than those of the control group,the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The recurrence rate of the observation group patients at 3 and 6 months after treatment was lower than that of the control group,but the difference was not statistically significant(P>0.05).Conclusion Insulin glargine+Metformin+combined estrogen ointment has a significant effect in the treatment of type 2 diabetes with senile Vaginitis,which can improve the thera‐peutic effect,clinical symptoms,blood sugar levels,vaginal environment,inflammatory factor levels,and has a high safety.
