TEN术后可塑性夹板对治疗儿童尺桡骨骨折的疗效及力学效应分析

摘要 目的分析弹性髓内针(titanic elastic nailing,TEN)术后可塑性夹板在儿童尺桡骨骨折中的应用价值及力学效应。方法选取2021年11月—2023年3月重庆市中医骨科医院的60例儿童尺桡骨骨折患儿作为研究对象,按抽签法随机分为治疗组和对照组,各30例。对照组通过弹性髓内针术后联合石膏外固定进行治疗,治疗组通过弹性髓内针术后联合可塑性夹板固定进行治疗,对比两组临床疗效及力学效应。结果治疗组愈合优良率(93.33%)高于对照组,差异有统计学意义(χ^(2)=5.455,P<0.05)。治疗组并发症发生率低于对照组,差异有统计学意义(P<0.05)。治疗4、6、8、12周后,治疗组X线评分均高于对照组,差异有统计学意义(P<0.05)。治疗组中立位、屈肘握拳位时扎带张力、扎带对小夹板的压力均高于对照组,差异有统计学意义(P<0.05)。结论与传统石膏相比,可塑性夹板在儿童尺桡骨骨折的治疗中具有明显的力学效应优势,而且固定效果良好,有利于预防术后并发症的发生,对患儿预后具有重要意义。 Objective To analyze the application value and mechanical effect of the plastic splint after the operation of the titanic elastic nailing(TEN)in children with ulnar and radius fractures.Methods Sixty children with ulna and radius fractures in Chongqing Orthopaedic Hospital of Traditional Chinese Medicine from November 2021 to March 2023 were selected as the study objects,and randomly divided into treatment group and control group according to the lottery method,with 30 cases in each group.The control group was treated by elastic intramedullary acupuncture com⁃bined with plaster external fixation,and the treatment group was treated by elastic intramedullary acupuncture com⁃bined with plastic splint fixation.The clinical and mechanical effects of the two groups were compared.Results The healing rate of the treatment group(93.33%)was higher than that of the control group,and the difference was statisti⁃cally significant(χ^(2)=5.455,P<0.05).The incidence of complication in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).After 4,6,8 and 12 weeks of treatment,the Xray scores in the treatment group were higher than those in the control group,and the difference was statistically sig⁃nificant(P<0.05).The tension of cable ties and the pressure of cable ties on the splint in the neutral position and bending elbow position of the treatment group were higher than those in the control group,the difference was statisti⁃cally significant(P<0.05).Conclusion Compared with traditional plaster,the plastic splint has obvious mechanical effect advantages in the treatment of radial and ulnar fractures in children,and the fixation effect is good,which is ben⁃eficial to the prevention of postoperative complications and has important significance for the prognosis of children.
