胰岛素泵对新诊断2型糖尿病患者血管内皮钙黏蛋白及内皮素的影响

Effects of insulin pump therapy on serum vascular endothelium-cadherin and endothelin 1 in patients with newly diagnosed type 2 diabetes mellitus

摘要 目的探讨胰岛素泵治疗对新诊断2型糖尿病(T2DM)患者血清血管内皮钙粘蛋白(VE-Cad及内皮素-1(ET-1)的影响。方法选取新诊断T2DM患者62例(DM组)及与其年龄、性别等指标相匹配的健康体检者35例(NC组)。测量体重指数(BMI)、血压[收缩压(SBR)、舒张压(DBR)],检测临床生化指标[三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、餐后2 h血糖(2hPG)],测定VE-Cad、ET-1水平。DM组经胰岛素泵治疗2周后复测上述指标。结果与NC组比较,DM组患者的VE-Cad、ET-1水平及HbA1c、FPG、2hPG、DBP、TG、TC、LDL-C均显著增高,HDL-C水平显著下降,差异均有统计学意义(P<0.05)。DM组经胰岛素泵治疗2周后,血清VE-Cad、ET-1水平均低于治疗前,差异均有统计学意义(P<0.01)。相关分析显示,血清VE-Cad水平与ET-1、FPG、2hPG、HbA1c、DBP、TG呈正相关(P<0.05)。结论胰岛素泵治疗能明显降低新诊断T2DM患者的血清VE-Cad、ET-1水平。 Objective To investigate the effects of insulin pump therapy on serum vascular endothelium-cadherin(VE-Cad)and endothelin 1(ET-1)levels in patients with newly diagnosed type 2 diabetes mellitus(T2DM).Methods A total of 62 patients with newly diagnosed T2DM were selected as DM group and 35 age-and sex-matched healthy volunteers were selected as negative control(NC)group.The body mass index(BMI),systolic and diastolic blood pressure(SBP and DBP),biochemical indexes(triacylglycerol[TG],total cholesterol[TC],high-density lipoprotein cholesterol[HDL-C],low-density lipoprotein cholesterol[LDL-C],fasting plasma glucose[FPG],hemoglobin A1C[HbA1c],2-hour postprandial plasma glucose[2hPG]),and serum VE-Cad and ET-1 were tested.After 2 weeks of insulin pump therapy,the above indexes were re-examined.Results Compared with those of NC group,serum VE-Cad and ET-1,HbA1c,FPG,2hPG,DBP,TG,TC and LDL-C in the DM group were significantly higher,and HDL-C was significantly lower in DM group(P<0.05).After 2 weeks of insulin pump therapy,serum VE-Cad and ET-1 were significantly lower than those before treatment(P<0.05).Correlation analysis showed that serum VE-Cad level was positively correlated with serum ET-1,FPG,2hPG,HbA1c,DBP and TG in patients with newly diagnosed T2DM(P<0.05).Conclusion Insulin pump therapy significantly lowers serum VE-Cad and ET-1 levels in patients with newly diagnosed T2DM.
