摘要
目的:分析和对比地氟烷与七氟烷吸入麻醉对单肺通气患者肺内分流的影响。方法:选择2022年1-12月贵州省人民医院收治的60例单肺通气患者作为研究对象,采用随机数表法将其分为对照组和观察组,各30例。对照组应用地氟烷麻醉,观察组应用七氟烷麻醉,对比两组麻醉前、麻醉30 min后肺内分流率、心率、平均动脉压等血流动力学指标水平、麻醉起效时间、术后麻醉苏醒时间及麻醉相关不良反应发生率。结果:麻醉前,两组血流动力学指标水平比较,差异无统计学意义(P>0.05);麻醉30 min后,对照组肺内分流率显著低于麻醉前,心率、平均动脉压显著高于麻醉前,差异有统计学意义(P<0.05);但观察组肺内分流率、心率、平均动脉压水平与麻醉前比较,差异无统计学意义(P>0.05);两组麻醉30 min后血流动力学指标比较,差异有统计学意义(P<0.05)。观察组麻醉起效时间、术后麻醉苏醒时间长于对照组,麻醉相关不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:与地氟烷相比,七氟烷吸入麻醉对单肺通气患者肺内分流和血流动力学的影响更小,不良反应更少,但七氟烷麻醉起效相对较慢,且麻醉苏醒时间较长。
Objective:To analyze and compare the effects of Desflurane and Sevoflurane inhalation anesthesia on intrapulmonary shunt in patients with one-lung ventilation.Method:A total of 60 patients with one-lung ventilation who admitted to Guizhou Provincial People's Hospital from January to December 2022 were selected as the study subjects,and they were divided into the control group and the observation group by random number table method,with 30 cases in each group.The control group was anesthetized with Desflurane and the observation group was anesthetized with Sevoflurane,and the levels of hemodynamic indexes such as intrapulmonary shunt,heart rate and mean arterial pressure before anesthesia and 30 min after anesthesia,the onset time of anesthesia,the recovery time of postoperative anesthesia and the incidence of anesthesia-related adverse reactions were compared between two groups.Result:Before anesthesia,there were no significant differences in hemodynamic indexes between two groups(P>0.05);after 30 minutes of anesthesia,the intrapulmonary shunt in the control group was significantly lower than that before anesthesia,and the heart rate and mean arterial pressure levels were significantly higher than before anesthesia,the differences were statistically significant(P<0.05),but there were no significant differences between intrapulmonary shunt,heart rate and mean arterial pressure in the observation group compared with those before anesthesia(P>0.05),there were significant differences in hemodynamic indexes between two groups(P<0.05).The onset time of anesthesia and the recovery time of postoperative anesthesia in the observation group were longer than those in the control group,and the incidence of anesthesia-related adverse reactions was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion:Compared with Desflurane,Sevoflurane inhalation anesthesia has less effect on intrapulmonary shunt and hemodynamics in patients with one-lung ventilation,and has fewer adverse reactions.However,Sevoflurane anesthesia has a relatively slow onset and a longer recovery time.
