摘要
目的探究3D打印经皮导板在改良经皮球囊扩张椎体后凸成形术治疗骨质疏松性椎体压缩性骨折(OVCF)中的临床效果。方法选取81例OVCF患者为研究对象,根据不同时期分为新鲜骨折组(n=40)和陈旧骨折组(n=41),均给予3D打印经皮导板技术辅助改良经皮球囊扩张椎体后凸成形术治疗。比较2组伤椎手术前后高度、手术前后VAS评分及ODI评分。结果新鲜骨折组伤椎前缘高度、中部高度高于陈旧骨折组,后凸高度明显小于陈旧骨折组(P<0.05);术后1 d和末次随访时,新鲜骨折组VAS评分显著小于陈旧骨折组(P<0.05);在末次随访时,新鲜骨折组ODI评分显著小于陈旧骨折组(P<0.05)。结论3D打印经皮导板在改良经皮球囊扩张椎体后凸成形术治疗骨质疏松性胸、腰椎骨折中对新鲜骨折疗效要明显优于对陈旧骨折的疗效,活动改善程度明显提高,在发现OVCF后应及时进行手术治疗。
Objective To investigate the clinical effect of 3D printed percutaneous guide plate navigation in the treatment of osteoporotic thoracolumbar fractures(OVCF)with improved percutaneous balloon expansion kyphoplasty.Methods A total of 81 OVCF patients were selected as the research subjects and divided into fresh fracture group(n=40)and old fracture group(n=41)according to different periods.The patients were all treated with 3D printed percutaneous navigation technology assisted with improved percutaneous balloon dilation and kyphoplasty.The height,VAS score and ODI score before and after surgery were compared between the two groups.Results The height of the anterior edge and middle part of the injured vertebra in the fresh fracture group was higher than that in the old fracture group,and the height of the kyphosis was significantly lower than that in the old fracture group(P<0.05).On the first day after surgery and at the last follow-up,the VAS score of the fresh fracture group was significantly lower than that of the old fracture group(P<0.05).At the last follow-up,the ODI score of the fresh fracture group was significantly lower than that of the old fracture group(P<0.05).Conclusion 3D printed percutaneous guide plate navigation is significantly more effective in the treatment of osteoporotic thoracolumbar fractures with improved percutaneous balloon expansion kyphoplasty for fresh fractures compared to old fractures.The degree of improvement in mobility is significantly higher,and timely surgical treatment should be performed after the discovery of OVCF.
