摘要
目的利用卫生技术评估技术分析山东省MRI配置的经济性与有效性,为山东省“十四五”期间MRI配置方案优化提供理论参考意见。方法通过德尔菲专家咨询法设计调研问卷,然后通过山东省大型医用设备配置管理系统,对山东省内16个地市的二甲及以上相关医院MRI的单台采购价、日开机时间、患者等待时间、单台维修成本、单台年保费用以及单台年均利润总额等参数进行调研分析,最后通过Excel 2019对MRI的分布情况、经济性、有效性等参数进行描述性统计分析。结果本研究共调研680台MRI,多数分布在三甲医院。通过分析发现,山东省内MRI明显存在分布地域不均衡现象,MRI人均台数最多的为济南,最少的为菏泽;患者平均等待时间最长的为青岛1.65 d,最短的为日照0.46 d;各地市MRI单台年均投资收益率为158.01%,最高的为滨州216.97%;年均检查结果阳性病例数为34.15万人次,最多的为济南80.89万人次;日平均开机时间为12.25 h,最长的为东营14.33 h;MRI日平均检查病例数为27.79例,日最大检查病例数最高的为济宁50.58例。结论本研究可为管理部门从总体上对医疗设备配置进行宏观调控提供参考借鉴,为医疗设备的合理配置研究提供相关参考数据,以及为区域内医疗资源的均衡发展发挥积极作用。
Objective To analyze the economy and effectiveness of MRI configuration in Shandong Province by using health technology assessment technology,and to provide theoretical suggestions for the optimization of MRI configuration scheme during the“14th Five-Year Plan”period in Shandong Province.Methods The questionnaire was designed by Delphi expert consultation method,and then the parameters such as single purchase price,daily start-up time,patient waiting time,single maintenance cost,annual premium cost and annual profit total of MRI were investigated and analyzed in 16 prefecture-level hospitals of grade A second class and above in Shandong Province through large-scale medical equipment configuration management system.Finally,Excel 2019 was used for descriptive statistical analysis of the distribution,economy and effectiveness of MRI.Results A total of 680 MRI sets were investigated in this study,most of which were distributed in grade A third class hospitals.The results showed that the distribution of MRI in Shandong Province was obviously unbalanced.The highest average number of MRI units per capita was in Jinan and the least was in Heze.Patients in Qingdao had the longest average waiting time of 1.65 days,and the shortest was Rizhao 0.46 days.The average annual return on investment of MRI was 158.01%,and the highest was 216.97%in Binzhou.The average number of positive cases was 341500,and the highest was 808900 in Jinan.The average daily startup time was 12.25 h,and the longest was 14.33 h in Dongying.The average number of MRI cases per day was 27.79,and the highest maximum number of MRI cases was 50.58 in Jining.Conclusion This study can provide reference for the management departments to conduct macro-control on the allocation of medical equipment in general,provide relevant reference data for the research on the reasonable allocation of medical equipment,and play a positive role in the balanced development of medical resources in the region.
