摘要
目的:探讨老年髋关节置换术患者术后康复运动管理中引入渐进模型的效果。方法:采用回顾性分析法,选择2020年6月至2022年6月南通大学附属南通第三医院收治的行老年髋关节置换术患者128例,将实施常规围手术期治疗及术后康复运动训练的64例患者作为对照组,将引入基于渐进模型的术后康复运动管理的64例患者作为研究组。对比2组康复情况、手术前后髋关节功能、步态指标、平衡能力及生活质量变化,并统计术后并发症发生率。结果:研究组术后恢复活动时间、髋关节恢复活动时间及住院时间均较对照组短(均P<0.05);术后3个月,研究组髋关节各维度评分、总分较对照组高,步宽较对照组短,步频较对照组高(均P<0.05);研究组睁眼支撑面稳定(eyes open when standing on firm support,EOFI)、闭眼支撑面稳定(eyes closed when standing on firm support,ECFI)、睁眼支撑面不稳(eyes open when standing on foam support,EOFO)、闭眼支撑面不稳(eyes closed when standing on foam support,ECFO)、综合值较对照组低(均P<0.05);研究组生活质量各维度评分、总分较对照组高,术后并发症总发生率较对照组低(均P<0.05)。结论:老年髋关节置换术患者术后康复运动管理中引入渐进模型效果显著,可改善髋关节功能及步态指标,提高平衡能力及生活质量,减少并发症的发生。
Objective:To explore the effect of introducing progressive model into the rehabilitation movement management in elderly patients undergoing hip replacement.Methods:A total of 128 elderly patients who underwent hip replacement in Nantong Third University Hospital from June 2020 to June 2022 were selected and randomly divided into a control group and a research group,each group with 64 patients.The control group received routine perioperative treatment and postoperative rehabilitation exercise training.On this basis,the research group was introduced postoperative rehabilitation exercise management based on a progressive model.The rehabilitation,hip function,gait index,balance ability and quality of life in the 2 groups before and after operation were compared,and the incidence of postoperative complications was counted.Results:The recovery time,hip joint recovery time and hospitalization time in the study group were shorter than those in the control group(all P<0.05).Three months after operation,the hip joint score and total score in the study group were higher than those in the control group,the step width was shorter and the step frequency was higher than those in the control group(all P<0.05).In the study group,the eyes open when standing on firm support(EOFI),eyes closed when standing on firm support(ECFI),eyes open when standing on foam support(EOFO),eyes closed when standing on foam support(ECFO)and their comprehensive values were lower than those in the control group(all P<0.05).The scores of all dimensions and total scores of life quality in the study group were higher than those in the control group,and the total incidence of postoperative complications was lower than that in the control group(all P<0.05).Conclusion:The introduction of progressive model into the rehabilitation movement management in elderly patients after hip replacement has a significant effect,which can improve hip function and gait indicators,improve balance ability and quality of life,and reduce complications.
