2021年浙江省孕妇碘营养水平横断面调查与分析 被引量:3

A cross-sectional study of iodine nutrition status in pregnant women in Zhejiang Province in 2021

摘要 目的了解浙江省碘缺乏病防治现状,掌握重点人群孕妇的碘营养水平。方法2021年在浙江省全部90个县(市、区)开展人群碘营养状况横断面调查。每个县(市、区)划为东、西、南、北、中5个片区,每个片区抽取1个乡镇(街道),每个乡镇(街道)抽取20名孕妇,采集其家中食用盐盐样和1次随机尿样,检测盐碘和尿碘含量,并对以上指标按照城市、农村,沿海、内陆,孕早期、孕中期、孕晚期进行分层分析。结果共检测9679份孕妇食用盐盐样,盐碘含量为(20.2±9.2)mg/kg,碘盐覆盖率为84.2%(8153/9679),合格碘盐食用率为80.4%(7785/9679)。其中,沿海地区孕妇合格碘盐食用率为69.4%(3175/4575),明显低于内陆地区[90.3%(4610/5104)];且食用非碘盐的比例[25.8%(1181/4575)]明显高于内陆地区[6.8%(345/5104)]。共检测9679份孕妇尿样,尿碘中位数为133.0μg/L(<150μg/L)。其中,城市孕妇尿碘中位数为129.8μg/L,低于农村(135.0μg/L);沿海地区孕妇尿碘中位数为126.0μg/L,低于内陆地区(140.3μg/L);而且随着孕期的推进,孕妇碘营养水平也逐渐降低(尿碘中位数:孕早期为137.0μg/L、孕中期为134.1μg/L、孕晚期为129.4μg/L)。结论2021年浙江省孕妇处于碘缺乏状态,且城市比农村、沿海比内陆存在更大风险。 Objective To evaluate the current situation of prevention and control of iodine deficiency disorders(IDD),and to describe the iodine nutrition level of pregnant women in key population in Zhejiang Province.Methods In 2021,a cross-sectional survey on iodine nutrition status of the population was conducted in all 90 counties(cities,districts)in Zhejiang Province.Each county(city,district)was divided into five districts:east,west,south,north,and central.One township(street)was selected from each district,and 20 pregnant women were selected from each township(street)to collect edible salt samples and one random urine sample for salt and urinary iodine testing.The above indicators were stratified and analyzed according to urban,rural,coastal,inland,early pregnancy,mid pregnancy,and late pregnancy.Results A total of 9679 pregnant women's salt samples were tested,with a salt iodine content of(20.2±9.2)mg/kg,an iodized salt coverage rate of 84.2%(8153/9679),and a qualified iodized salt consumption rate of 80.4%(7785/9679).Among them,the qualified iodized salt consumption rate of pregnant women in coastal areas was 69.4%(3175/4575),significantly lower than that in inland areas[90.3%(4610/5104)];and the proportion of non iodized salt consumption in coastal areas[25.8%(1181/4575)]was significantly higher than inland areas[6.8%(345/5104)].A total of 9679 pregnant women's urine samples were tested,with a median urinary iodine of 133.0μg/L(<150μg/L).In urban areas,the median urinary iodine(129.8μg/L)was lower than that in rural areas(135.0μg/L).Likewise,the median urinary iodine was 126.0μg/L,lower than inland areas(140.3μg/L).And as pregnancy progressed,the iodine nutrition level of pregnant women gradually decreased(the median urinary iodine:137.0μg/L in early pregnancy,134.1μg/L in mid pregnancy,and 129.4μg/L in late pregnancy).Conclusion In 2021,pregnant women in Zhejiang Province are in a state of iodine deficiency,and there is a greater risk in urban areas compared to rural areas and coastal areas compared to inland areas.
