Ki-67、CyclinD-1及STAT3蛋白在乳腺叶状肿瘤组织中的表达及其临床意义分析 被引量:1

摘要 目的探究Ki-67、细胞周期蛋白D-1(CyclinD-1)及信号转导子和转录激活子3(STAT3)蛋白在乳腺叶状肿瘤组织中的表达及其临床意义。方法回顾性选取2019年1月至2022年1月琼海市人民医院收治的75例乳腺叶状肿瘤患者纳入研究,经术后病理检查结果为恶性的68例患者入组。取患者肿瘤组织及其癌旁组织,采用免疫组织化学法对患者的Ki-67、CyclinD-1在组织中的表达情况进行检测;并采用蛋白质印迹法对患者组织中的STAT3蛋白表达情况进行检测;分析患者肿瘤组织中Ki-67、CyclinD-1及STAT3蛋白表达情况与患者临床病理间的关系,并分析其表达对患者乳腺恶性叶状肿瘤的诊断价值。结果乳腺叶状肿瘤组织中Ki-67、CyclinD-1、STAT3蛋白表达阳性率分别为30.88%、72.06%、22.06%,均高于癌旁组织(4.41%、16.18%、2.94%),差异均有统计学意义(P<0.05)。Ki-67阳性表达在年龄大小、肿瘤部位、组织学分级、是否绝经等方面比较,差异均无统计学意义(P>0.05),但患者临床分期为Ⅲ期的阳性率为55.56%,明显高于临床分期为Ⅰ~Ⅱ期患者(14.63%),差异有统计学意义(P<0.05)。CyclinD-1阳性表达在是否绝经、肿瘤部位、年龄大小等方面比较,差异均无统计学意义(P>0.05),但患者临床分期为Ⅲ期、组织学分级为3级的阳性率分别为82.22%、83.78%,均明显高于临床分期为Ⅰ~Ⅱ期、组织学分级为1~2级患者(52.17%、58.06%),差异均有统计学意义(P<0.05)。STAT3蛋白阳性表达在年龄大小、肿瘤部位、绝经情况等方面比较,差异均无统计学意义(P>0.05),但临床分期为Ⅲ期、组织学分级为3级的阳性率分别为54.55%、40.00%,均明显高于临床分期为Ⅰ~Ⅱ期、组织学分级为1~2级患者(6.52%、11.63%),差异均有统计学意义(P<0.05)。Ki-67、CyclinD-1、STAT3蛋白阳性表达诊断患者乳腺叶状肿瘤的受试者工作特征(ROC)曲线下面积(AUC)分别为0.732、0.879和0.721,95%CI分别为0.538~0.746、0.699~0.898、0.500~0.743。结论乳腺组织中Ki-67、CyclinD-1、STAT3蛋白过表达可能与患者乳腺叶状肿瘤有关,对其诊断具有一定价值。 Objective To explore the expression and clinical significance of Ki-67,cyclin D-1(CyclinD-1)and signal transducer and activator of transcription 3(STAT3)protein in lobe tumor tissue.Methods A total of 75 patients with breast phyllodes tumor admitted to Qionghai People's Hospital were retrospectively selected into this study.After postoperative pathological examination,68 patients were enrolled(the pathological examination results were malignant).The tumor tissue and its adjacent tissues of the patient were collected,and the expression of Ki-67 and CyclinD-1 in the tissue was detected by immunohistochemistry.And the expression of STAT3 protein in the patient's tissue was detected by Western blotting.The relationship between the expression of Ki-67,CyclinD-1,and STAT3 proteins in the tumor tissue of patients and their clinical pathology was analyzed,and their diagnostic value for malignant phyllodes tumors of the breast in patients was analyzed.Results The positive rates of Ki-67,CyclinD-1,and STAT3 protein expression in breast phyllodes tumor tissues were 30.88%,72.06%,and 22.06%,respectively,which were higher than those in adjacent tissues(4.41%,16.18%,2.94%),and the differences were statistically significant(P<0.05).There was no statistically significant difference in Ki-67 positive expression in terms of age,tumor location,histological grading,and menopause(P>0.05),but the positive rate of patients with clinical stageⅢwas 55.56%,which was significantly higher than that of patients with clinical stageⅠ-Ⅱ(14.63%),and the difference was statistically significant(P<0.05).The positive expression of CyclinD-1 was not different in menopause,tumor site,and age(P>0.05),but the positive rates of patients with clinical stage and histological grade grade 3 were 82.22%,83.78%,which were significantly higher than those of patients with clinical stageⅠ-Ⅱand histological grade 1 to 2(52.17%,58.06%),and the differences were statistically significant(P<0.05).The positive expression of STAT3 varied significantly in age,tumor site and menopause(P>0.05),but the positive rates of clinical stageⅢand histological grade 3 were 54.55%,40.00%,which were significantly higher than those of patients with clinical stageⅠ-Ⅱand grade 1 to 2(6.52%,11.63%),and the differences were statistically significant(P<0.05).The receiver operating characteristic area under the curve for Ki-67,CyclinD-1 and STAT3 protein expression was 0.732,0.879 and 0.721,respectively,and the 95%CI was 0.538-0.746,0.699-0.898,0.500-0.743,respectively.Conclusion The overexpression of Ki-67,CyclinD-1 and STAT3 proteins in breast tissue may be related to lobe tumors of patients and have certain value for their diagnosis.
