摘要
代谢综合征(metabolic syndrome,MS)是以腹部肥胖、胰岛素抵抗、高血压和高脂血症为主要临床特征的一组复杂的代谢紊乱证候群。随着西方生活方式在全球范围内的传播,发展中国家的MS发病率急剧上升,MS现在已成为一个真正的全球性问题。过多的能量摄入和伴随的肥胖是该综合征的主要驱动因素。生活方式干预可以逆转代谢风险因素,但有时可能需要药物治疗或减肥手术来控制更明显的风险因素。目前临床对于MS的治疗,仅限于针对各个病理特征的单独或联合药物治疗,如抗肥胖药物、噻唑烷二酮类、二甲双胍、他汀类药物等。黄连温胆汤首见于由清代陆廷珍编撰的《六因条辨》中,由黄连、半夏、茯苓、陈皮、枳实、竹茹、炙甘草、生姜8种药物组成,能够清热燥湿、理气化痰、和胃利胆,具有显著的降脂、降糖、抗炎等药理作用,在治疗MS上具有多途径、多靶点、多机制协同调控的特点。因此,文章将从黄连温胆汤单味药有效成分及成方所涉及相关机制如炎症、氧化应激等方面对MS的防治进行综述,为推进MS的实验研究及临床治疗提供科学依据。
Metabolic syndrome(MS)is a complex group of metabolic disorder syndromes with abdominal obesity,insulin resistance,hypertension and hyperlipidemia as the main clinical features.With the global spread of the Western way of life,the incidence of MS in developing countries has risen sharply,and MS is now a truly global problem.Excessive energy intake and concomitant obesity are major drivers of this syndrome.Lifestyle interventions can reverse metabolic risk factors,but sometimes medication or bariatric surgery may be required to control more pronounced risk factors.The current clinical treatment of MS is limited to single or combined drug therapy for each pathological feature,such as anti-obesity drugs,thiazolidinediones,metformin,and statins.Huanglian Wendan Decoction(黄连温胆汤)was first seen in the Six Factors Discrimination compiled by LU Tingzhen in the Qing dynasty.Regulating Qi and resolving phlegm,and stomach and gallbladder,have significant pharmacological effects such as lipid-lowering,hypoglycemic,anti-inflammatory,etc.,and have the characteristics of multi-channel,multi-target,multimechanism coordinated regulation in the treatment of MS.Therefore,this article will review the prevention and treatment of MS from the effective components of Huanglian Wendan Decoction and the related mechanisms involved in its formulations,such as inflammation and oxidative stress,to provide a scientific basis for advancing the experimental research and clinical treatment of MS.
