摘要
肝衰竭(liver failure,LF)是各种病因引起的肝细胞死亡,并以肝功能急剧恶化为主要表现的一类疾病。LF发病率高,病情进展迅速,病死率高,是目前国内外急需解决的临床重难点。JAK/STAT信号通路参与肝纤维化、肝硬化、LF、肝癌等疾病发病的重要环节,尤其与LF的发生发展紧密相关。检索大量文献发现Janus激酶/信号转导与转录激活子(JAK/STAT)信号通路及其下游关键靶点通过调控细胞凋亡、细胞自噬、氧化应激、炎症反应参与LF的病理生理过程。中医药作为传统医学瑰宝,以多通路、多靶点、多分子等优势广泛应用于LF的临床治疗。目前中医药调节JAK/STAT信号通路治疗LF的国内外相关基础研究日益增长,具有良好的研究价值。作者通过阐述JAK/STAT信号通路及其下游靶点与LF的关系,并对中医药调控JAK/STAT信号通路治疗LF的疗效作出归纳总结,旨在进一步为LF的防治提供新思路和治疗策略。
Liver failure(LF)is a kind of disease characterized by the death of liver cells caused by various causes and the rapid deterioration of liver function.The incidence of LF is high,the disease progresses rapidly,and the mortality rate is high,which is an urgent clinical difficulty to be solved at home and abroad.JAK/STAT signaling pathway is involved in the pathogenesis of liver fibrosis,cirrhosis,LF,liver cancer and other diseases,especially closely related to the occurrence and development of LF.The Janus kinase/signal transducer and activator of transcription(JAK/STAT)signaling pathway and its downstream key targets are involved in the pathophysiological process of LF by regulating apoptosis,autophagy,oxidative stress,and inflammatory response.As a treasure of traditional medicine,traditional Chinese medicine has been widely used in the clinical treatment of LF due to its advantages of multi-pathway,multi-target and multi-molecule.At present,the basic research of traditional Chinese medicine regulating JAK/STAT signaling pathway in the treatment of LF is growing at home and abroad,which has good research value.This article reviews the relationship between JAK/STAT signaling pathway and its downstream targets and LF,and summarizes the efficacy of traditional Chinese medicine in the treatment of LF by regulating JAK/STAT signaling pathway,so as to provide new ideas and therapeutic strategies for the prevention and treatment of LF.
