摘要
张磊老师是第三届国医大师,理验俱丰、医术精湛,行医近70年,擅用临证八法治疗内科杂病。作者有幸成为张磊国医大师第一批传承弟子,于跟师侍诊时发现其常于病案中亲自书写“沉滞脉”,临床可见于肝郁气滞、气郁化火、痰湿中阻、情志抑郁、浊邪内生、瘀血阻络、经络湮瘀、少阳经气失枢等几种证型,多见于年轻人中亚健康状态或代谢综合征患者,相对轻的病人偏多,但亦可见于年迈体虚,合并多种慢性并发症的老年患者。“沉滞脉”病案涉及病种包括呕吐、胃痛、胃胀、胁痛、口苦、痞证、失眠、心悸、中风、痹证、皮肤病、月经病等多种疾病。在选方上,张磊老师广泛应用临证八法处方用药,灵活选用经方、时方、自拟经验方,脉证结合,组方用药,临床效验。
Tutor ZHANG Lei is the third master of traditional Chinese medicine,with abundant physical examination and exquisite medical skills,has been practicing medicine for nearly 70 years,using the eight methods of clinical evidence to treat miscellaneous diseases in internal medicine.Having the honor to become the first batch of inherited disciples of master ZHANG Lei,we find that he often personally writes“deep and stagnant pulse”in the cases,which can be clinically seen in stagnation of liver Qi,Qi depression transforming into fire,phlegm-damp obstruction,emotional depression,turbidity endogenous,blood stasis obstruction,meridian annihilation,Shaoyang meridian Qi loss and other types,mostly seen in young people with sub-healthy state or patients with metabolic syndrome,more relatively mild patients,but also can be seen in the elderly patients with weakness,combined with a variety of chronic complications.The“deep and stagnant pulse”cases involve a variety of diseases including vomiting,stomach pain,stomach distension,flank pain,bitter mouth,diarrhea,insomnia,palpitations,stroke,paralysis,skin diseases,menstrual diseases and so on.In the selection of prescriptions,tutor ZHANG Lei widely uses the eight methods of clinical evidence to prescribe medications,flexibly selects the classical prescriptions,non-classical prescriptions and the self-made prescriptions,and combines the pulse with the syndromes,and the prescriptions have clinical effect.
