摘要
目的 验证运用计划-执行-检查-处理(PDCA)循环法提高住院患儿抗菌药物治疗前病原学送检率的效果。方法 于2021年6月-2021年8月运用PDCA循环法对住院患儿抗菌药物治疗前病原学送检过程进行规范化管理,分别调查2021年1月-2021年3月;2021年9月-2021年11月;2022年2月-2022年4月三个时间段住院患儿抗菌药物治疗前病原学送检情况并进行对比。结果 PDCA循环法实施后,住院患儿抗菌药物治疗前病原学总体送检率由38.22%提高至87.86%,半年后送检率仍可达到61.26%,差异均具有统计学意义(P<0.05);除特殊级抗菌药物送检率差异无统计学意义外,非限制级抗菌药物及限制级抗菌药物送检率均在PDCA循环后得到了提高,分别由38.24%和36.59%提高至85.87%和91.89%,半年后仍维持在56.94%和68.75%,差异均具有统计学意义(P<0.05)。结论 在科室管理中利用PDCA循环,通过完善制度,制订标准流程,加强培训等方式可有效地提高住院患儿抗菌药物治疗前病原学送检率,对保障住院患儿合理应用抗菌药物具有实践意义。
OBJECTIVE To verify the effect of plan-do-check-action(PDCA)cycle on improving the pathogenic examination rate before antibiotic therapy in hospitalized children.METHODS From Jun 2021 to Aug 2021,PDCA cycle was used to conduct standardized management of the pathogenic examination process of hospitalized children before antibiotic therapy.The etiological examination of hospitalized children before antibacterial treatment in three time periods(Jan 2021-Mar 2021,Sep 2021-Nov 2021,Feb 2022-Apr 2022)was investigated and compared.RESULTS After the implementation of PDCA cycle,the pathogen detection rate increased from 38.22%to 87.86%,and that still reached 61.26% half a year later;the differences were significant(P<0.05).After PDCA cycle,the detection rates of unrestricted antibiotics and restricted antibiotics increased from 38.24% and 36.59%to 85.87% and 91.89%,respectively,and maintained 56.94% and 68.75% half a year later;the differences were significant(P<O.05).There was no significance in the detection rate of special grade antibiotics.CONCLUSION The use of PDCA cycle in department management can effectively improve the pathogen detection rate of hospitalized children before antimicrobial treatment by improving the system,formulating standard procedures,and strengthening training,which has practical significance for ensuring the rational use of antibacterial drugs in hospitalized children.
