摘要
目的:观察槐杞黄颗粒联合醋酸泼尼松、他克莫司治疗儿童紫癜性肾炎(HSPN)的疗效。方法:将112例HSPN患儿随机分为观察组和对照组各56例(最终实际纳入观察组53例,对照组51例)。对照组患儿给予醋酸泼尼松、他克莫司口服,观察组在对照组的基础上加用槐杞黄颗粒口服,4周为1疗程,连续用药3个疗程。比较两组的总有效率,血肌酐(SCr)水平、尿中β_(2)-微球蛋白(β_(2)-MG)含量、24 h尿蛋白量、尿沉渣红细胞数量、血清中干扰素-γ(IFN-γ)水平、白介素-6(IL-6)水平及不良反应。结果:治疗后,观察组临床治疗总有效率、IFN-γ水平高于对照组,SCr、β_(2)-MG、24 h尿蛋白量、尿沉渣红细胞计数、IL-6水平低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:槐杞黄颗粒联合醋酸泼尼松、他克莫司治疗儿童HSPN疗效确切,能保护肾功能,调控IFN-γ、IL-6水平,纠正免疫失衡状态,未明显增加不良反应。
Objective:To observe the curative effect of Huaiqihuang granules combined with prednisone acetate and tacrolimus in the treatment of children with Henoch-Schönlein purpura nephritis(HSPN).Methods:One hundred and twelve children with HSPN were divided into an observation group and a control group with 56 cases each(eventually,53 cases in the observation group and 51 cases in the control group were actually included).The control group was given prednisone acetate and tacrolimus orally,while the observation group was given Huaiqihuang granules orally on the basis of the control group.A course of treatment was four weeks and the total medication was continuously lasted for 3 courses.The total effective rate,serum creatinine(SCr)level,urinary β_(2)-microglobulin(β_(2)-MG)content,urinary protein content and urinary sludge red blood cell count at 24 h,serum interferon-g(IFN-γ)and interleukin-6(IL-6)contents and the incidence of adverse reactions were compared between the two groups.Results:After the treatment,the clinical efficacy and the level of IFN-γ were higher and the levels of SCr,β_(2)-MG,24 h urine protein and IL-6 and the urine sludge red blood cell count were lower in the observation group than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Huaiqihuang granule combined with prednisone acetate and tacrolimus has a definite curative effect in the treatment of children with HSPN and can protect renal function,regulate levels of IFN-γand IL-6 and correct immune imbalance without increase of adverse reactions.
