摘要 目的分析ABO疑难血型的原因及鉴定方法,确保临床输血安全。方法应用ABO疑难血型三步分析法,对本院2020年6月至2022年12月本院接收的110例ABO正反定型不一致标本进行分析鉴定。结果110例ABO正反定型不一致标本中,人为因素造成的有18例(16.36%);由于疾病本身或治疗因素造成的有92例(83.64%)。人为因素中,人员技术问题占比27.78%、未按规程操作占比22.22%、标本污染占比22.22%、标本标签错误占比11.11%、离心机问题占比11.11%、试剂过期占比5.56%。92份因疾病本身或治疗因素中,抗原因素28例,占比30.43%;抗体因素64例,占比67.57%。结论应用ABO疑难血型三步分析法能快速有效的解决疑难血型的鉴定,确保临床输血安全。 Objective To analyze the causes and identification methods of ABO difficult blood types,and ensure the safety of clinical blood transfusion.Methods The ABO difficult blood group three-step analysis method was used to analyze and identify 110 samples of ABO positive and negative typing inconsistency received by our hospital from June 2020 to December 2022.Results Among the 110 samples with inconsistent ABO positive and negative typing,18 cases(16.36%)were caused by human factors;92 cases(83.64%)were caused by the disease itself or treatment factors.Among human factors,personnel technical issues accounted for 27.78%,non adherence to procedures accounted for 22.22%,sample contamination accounted for 22.22%,sample label errors accounted for 11.11%,centrifuge issues accounted for 11.11%,and expired reagents accounted for 5.56%.Among 92 cases due to the disease itself or treatment factors,pregnancy history accounted for 33.70%,blood transfusion history accounted for 15.22%,and leukemia accounted for 10.87%.Conclusion The application of ABO three-step analysis method for difficult blood types can quickly and effectively solve the identification of difficult blood types,ensuring the safety of clinical blood transfusion,and is worthy of our promotion and application.
