CYP2C19、APOE以及SCLO1B1基因与对比剂肾病发生的相关性研究

摘要 目的探讨氯吡格雷(CYP2C19)基因、他汀基因(APOE基因以及SCLO1B1基因)与对比剂肾病发生的相关性。方法选取包头市中心医院心内科2021年10月至2022年10月收治的行冠状动脉CTA及PCI的患者500例,依据注射对比剂前后48~72 h肌酐水平分为对比剂肾病组(CIN组,49例)和非对比剂肾病组(非CIN组,451例)。比较两组患者一般生物学资料、血脂、尿酸、糖化血红蛋白、肌钙蛋白I、血浆NT-proBNP浓度、血浆CK-MB浓度和氯吡格雷基因、他汀基因、对比剂用量相关指标等。结果氯吡格雷基因慢代谢组CIN发病率高于快代谢组,差异存在统计学意义(P=0.001,P<0.0167);E2/E2,E2/E3组CIN发病率高于E2/E4,E3/E3组,差异有统计学意义(P=0.002,P<0.0167)。结论(1)氯吡格雷基因慢代谢型相比快代谢人群更容易发生对比剂肾病;(2)APOE基因的E2/E2,E2/E3组人群比E2/E4,E3/E3人群更容易发生对比剂肾病。 Objective:To investigate the correlation between clopidogrel(CYP2C19)gene,statin gene(APOE gene and SCLO1B1 gene)and contrast nephropathy.Methods A total of 500 patients with coronary CTA and PCI admitted to the cardiology Department of Baotou Central Hospital from October 2021 to October 2022 were selected and divided into contrast nephropathy group(CIN group)(49 cases)and non-contrast nephropathy group(non-CIN group)(451 cases)according to creatinine levels 48-72 hours before and after injection of contrast nephropathy.General biological data,blood lipid detection,uric acid,glycated hemoglobin,troponin I,plasma NT-proBNP concentration,plasma CK-MB concentration,clopidogrel(CYP2C19)gene,statin gene(APOE gene and SLCO1B1 gene),and relative indicators of contrast agent dosage were compared between the two groups.Results The incidence of CIN in the slow metabolized group of clopidogrel gene was higher than that in the fast metabolized group,the difference was statistically significant(P=0.001,P<0.0167).The incidence of CIN in E2/E2 and E2/E3 groups was higher than that in E2/E4 and E3/E3 groups,and the difference was statistically significant(P=0.002,P<0.0167).Conclusion 1.Slow metabolizers of clopidogrel gene are more prone to contrast nephropathy than fast metabolizers.2.The E2/E2,E2/E3 groups of APOE gene are more prone to contrast nephropathy than E2/E4 and E3/E3 groups.
