摘要
目的 探讨腹腔镜经腹腹膜前疝修补术(TAPP)联合无钉合补片治疗腹股沟疝患者的效果。方法 选取2018年1月至2021年6月我院收治的160例腹股沟疝患者作为研究对象,依据入院编号的奇偶性将其分为观察组与对照组,各80例。观察组实施TAPP联合无钉合补片治疗,对照组实施TAPP联合传统钉合补片治疗。比较两组的治疗效果。结果 观察组的治疗总有效率高于对照组(P<0.05)。治疗后1周,观察组的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及C-反应蛋白(CRP)水平均低于对照组,白细胞介素-10(IL-10)水平高于对照组(P<0.05)。观察组的手术时间、术后排气时间及住院时间均短于对照组,术中失血量少于对照组(P<0.05)。手术开始后1 h(T_(1))~手术结束后3 d(T_(3)),观察组的皮质醇(Cor)、丙二醛(MDA)水平及心率(HR)、平均动脉压(MAP)均优于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论 TAPP联合无钉合补片治疗腹股沟疝患者可以显著提高临床效果,减轻炎症反应、应激反应,稳定生命体征,改善手术指标及康复指标,并降低并发症发生率。
Objective To investigate the effect of laparoscopic transabdominal preperitoneal hernia repair(TAPP)combined with non-nail patch in the treatment of patients with inguinal hernia.Methods A total of 160 patients with inguinal hernia admitted in our hospital from January 2018 to June 2021 were selected as the research objects.According to the parity of admission number,the patients were divided into observation group and control group,with 80 cases in each group.The observation group was treated with TAPP combined with non-nail patch,and the control group was treated with TAPP combined with traditional nail patch.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).One week after treatment,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)in the observation group were lower than those in the control group,and the level of interleukin-10(IL-10)was higher than that in the control group(P<0.05).The operation time,postoperative exhaust time and hospitalization time of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group(P<0.05).From 1 h after the beginning of the operation(T_(1))to 3 d after the end of the operation(T_(3)),the cortisol(Cor),malondialdehyde(MDA)levels,heart rate(HR)and mean arterial pressure(MAP)in the observation group were better than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion TAPP combined with non-nail patch in the treatment of inguinal hernia patients can significantly improve the clinical effect,reduce inflammatory response,stress response,stabilize vital signs,improve surgical indicators and rehabilitation indicators,and reduce the incidence of complications.
