摘要
目的Tfr细胞及炎性因子在脓毒症患者中的表达及对预后的影响。方法选取2020年6月—2022年6月在本院进行脓毒症治疗的100例患者为脓毒症组,另取100例来本院进行健康体检者作为健康对照组。采用酶联免疫法检测血清中抑炎细胞因子(IL-4、IL-10、IL-35)、促炎细胞因子(IL-6、IL-1β);肿瘤坏死因子(TNF-α),流式细胞仪检测Tfr细胞标志物(CD3^(+)、CD4^(+)、CD4^(+)CD25^(+)),logistic分析脓毒症患者不良预后产生的因素。结果与健康组比较,脓毒症组患者血清中促炎细胞因子IL-6、IL-1β、TNF-α及Tfr细胞标志物CD3^(+)、CD4^(+)、CD4^(+)CD25^(+)升高,抑炎细胞因子IL-4、IL-10、IL-35降低(均P<0.05)。预后分析中发现,预后不良的脓毒症患者的抑炎细胞因子IL-4、IL-10、IL-35降低,促炎细胞因子IL-6、IL-1β、TNF-α升高及Tfr细胞标志物CD3^(+)、CD4^(+)、CD4^(+)CD25^(+)水平显著升高(均P<0.05);多因素logistic回归分析结果中,IL-4、IL-10、IL-35、IL-6、IL-1β、TNF-α、CD3^(+)、CD4^(+)、CD4^(+)CD25^(+)均是导致脓毒症患者预后不良的因素。结论Tfr细胞以及炎性因子的异常不仅对增加脓毒症患者的疾病反应具有重要促进作用,还会在一定程度上影响患者的预后。
Objective This paper aims to investigate the expression of Tfr cells and inflammatory factors in patients with sepsis and their effect on prognosis.Methods A total of 100 patients who received sepsis treatment in our hospital from June 2020 to December 2022 were enrolled into this study as the sepsis group,and another 100 patients who came to our hospital for physical examination were selected as the healthy control group.Serum anti-inflammatory cytokines(IL-4,IL-10,IL-35)and proinflammatory cytokines(IL-6,IL-1β,TNF-α)were detected by enzyme-linked immunosorbent assay.Tfr cell markers(CD3^(+),CD4^(+),CD4^(+)CD25^(+))were detected by flow cytometry.Logistic analysis was used to analyze the factors of poor prognosis in patients with sepsis.Results Compared with healthy group,serum proinflammatory cytokines IL-6,IL-1β,TNF-α and Tfr cell markers CD3^(+),CD4^(+),CD4^(+)CD25^(+)were increased in sepsis group,while anti-inflammatory cytokines IL-4,IL-10 and IL-35 were decreased(P<0.05).Prognostic analysis showed that the anti-inflammatory cytokines IL-4,IL-10 and IL-35 were decreased in patients with sepsis with poor prognosis,while the pro-inflammatory cytokines IL-6,IL-1β and TNF-α were increased and the levels of Tfr cell markers CD3^(+),CD4^(+)and CD4^(+)CD25^(+)were significantly increased(P<0.05).Multivariate logistic regression analysis showed that IL-4,IL-10,IL-35,IL-6,IL-1β,TNF-α,CD3^(+),CD4^(+),CD4^(+)CD25^(+)were all factors leading to poor prognosis in patients with sepsis.Conclusion The abnormalities of Tfr cells and inflammatory factors not only play an important role in promoting the disease response of patients with sepsis,but also affect the prognosis of patients to a certain extent.
