胃肠道间质瘤组织病理特征下沉默信息调节因子1的表达及其对预后的影响 被引量:1

Expression of silencing information regulatory factor 1 in pathological characteristics of gastrointestinal stromal tumors and its impact on prognosis

摘要 目的探讨胃肠道间质瘤(Gastrointestinal stromal tumors,GIST)组织病理特征下沉默信息调节因子1(Silent information regulator 1,SIRT1)的表达及其对预后的影响。方法回顾性分析黄山首康医院接受手术治疗的52例GIST患者临床资料,分析SIRT1的表达与GIST患者病理特征、生存状况的关系以及GIST患者预后的影响因素。结果肿瘤组织中SIRT1蛋白阳性表达率显著高于肿瘤旁组织中SIRT1蛋白阳性表达率(P<0.05)。SIRT1阳性组肿瘤直径显著大于阴性组。SIRT1阳性组中核分裂象<5个/50HPF、Ki-67增殖指数<5%、危险程度极低/低的患者占比显著低于阴性组,而≥5个/50HPF、Ki-67增殖指数≥5%、危险程度中/高患者占比显著高于阴性组(P均<0.05)。SIRT1阴性表达患者存活率显著高于阳性表达患者(P<0.05)。SIRT1阳性表达、Ki-67增值指数≥5%、中/高危险程度是GIST患者预后不良的影响因素(P均<0.05)。结论GIST患者临床病理特征影响SIRT1的表达,且SIRT1阳性表达可导致GIST患者预后不良。 Objective Exploring the expression of silencing information regulator 1(SIRT1)under the pathological characteristics of gastrointestinal stromal tumors(GIST)and its impact on prognosis.Methods The clinical data of 52 GIST patients who received surgical treatment in the hospital were retrospectively analyzed to explore the relationship between the expression of SIRT1 and the pathological characteristics and survival status of GIST patients,and to analyze the factors affecting the prognosis of GIST patients.Results The positive expression rate of SIRT1 protein in tumor tissue was significantly higher than that in adjacent tissues(P<0.05).The tumor diameter in the SIRT1 positive group was significantly larger than that in the negative group.The proportion of the patients with mitotic figures<5/50HPF,Ki-67 proliferation index<5%,and extremely low/low risk in the SIRT1 positive group was significantly lower than that in the negative group,while the proportion of the patients with≥5/50HPF,Ki-67 proliferation index≥5%,and medium/high risk was significantly higher than that in the negative group(P<0.05).The survival rate of SIRT1 negative expression patients were significantly higher than that of posi-tive expression patients(P<0.05).Positive expression of SIRT1,Ki-67 value-added index≥5%and moderate/high risk were influencing factors for poor prognosis in GIST patients(P<0.05).Conclusion The clinical pathological characteristics of GIST patients affect the expression of SIRT1,and positive expression of SIRT1 can lead to poor prognosis in GIST patients.
