摘要
目的 基于不同频次“针刺时效性”,观察电针介入时机对压力性尿失禁(stress urinary incontinence,SUI)大鼠膀胱尿道功能的影响。方法 60只雌性大鼠随机分为正常组,SUI模型组,电针治疗(电针)A组、B组、C组,每组12只。除正常组大鼠外均进行卵巢切除和阴道扩张造模。造模成功后,每组大鼠均采用0.9%氯化钠溶液+无菌美兰混合液进行膀胱灌注,检测尿动力学相关指标。随后用电针治疗A组、B组、C组大鼠“次髎/中髎”穴,分别进行隔2日1次、隔1日1次、每日1次治疗。2周治疗结束后,再次观察各组大鼠尿动力学变化。结果 与正常组比较,模型组大鼠漏尿点压、最大膀胱容量、排尿时间间隔显著下降(P<0.01);与SUI模型组比较,电针各组大鼠漏尿点压、最大膀胱容量、排尿时间间隔显著升高(P<0.05)。与电针A组比较,电针B组、C组大鼠漏尿点压升高(P<0.01)、大鼠排尿时间间隔延长(P<0.05),电针C组大鼠最大膀胱容量显著上升(P<0.01);与电针B组比较,电针C组大鼠漏尿点压、最大膀胱容量升高(P<0.05),排尿时间间隔延长(P<0.01)。结论 不同频次电针均能改善SUI大鼠膀胱尿道功能,但每日电针方案效果优于其他电针方案;每日电针主要通过缩短间隔时间、增加电针时效性来提高电针对SUI大鼠膀胱尿道功能的治疗作用。本研究进一步佐证了不同频次电针与针刺效应之间的正相关关系。
Objective To observe the effect of electroacupuncture intervention timing on bladder and urethral function in rats with stress urinary incontinence(SUI)based on the"time effect of acupuncture"at different frequencies.Methods 60 female rats were randomly divided into normal group,SUI model group,electroacupuncture group A,B and C,12 rats in each group.Except for the normal group,ovariectomy and vaginal dilation were performed for rats’modeling.After successful modeling,0.9%normal saline+sterile methylene blue mixture was used for intravesical instillation,and the urodynamic parameters of the bladder of rats under normal physiological state in each group were detected.Subsequently,the rats in group A,B,and C were treated with electroacupuncture at"BL32/BL33"acupoint.Group A received treatment every other day,group B received treatment every day with a one-day interval,and group C received daily treatment.The treatment lasted for 2 weeks.After the treatment,the urodynamic changes of the rats in each group were observed again.Results Compared with the normal group,the leakage point pressure,maximum bladder capacity,and voiding time interval in the rats of model group were all decreased(P<0.01).Compared with the SUI model group,the leakage point pressure,maximum bladder capacity,and voiding time interval in the rats of all electroacupuncture groups increased significantly(P<0.05).Compared with group A,the leakage point pressure of the rats increased significantly(P<0.01),while the voiding time interval was prolonged in the rats of group B and C(P<0.05),the maximum bladder capacity of the rats in group C increased significantly(P<0.01).Compared with group B,the leakage point pressure and maximum bladder capacity of the rats increased(P<0.05),while the voiding time interval of rats in group C was significantly prolonged(P<0.01).Conclusion Electroacupuncture at different frequencies can improve bladder and urethral function in SUI rats.However,the daily electroacupuncture protocol demonstrated superior effectiveness compared to other electroacupuncture protocols.The daily electroacupuncture primarily improved the therapeutic effect on bladder and urethral function in SUI rats by shortening the interval and enhancing the timeliness of electroacupuncture treatment.This study further supported the positive correlation between different frequencies of electroacupuncture and the acupuncture effect.
