摘要
目的:探讨糖皮质激素治疗早产儿支气管肺发育不良(BPD)的临床效果。方法:选取2020年6月—2022年6月新余市人民医院新生儿重症监护病房(NICU)收治的72例BPD患儿,按随机数字表法分为两组,各36例。对照组予以常规治疗,观察组加用糖皮质激素治疗。比较两组BPD严重程度、临床指标、血气指标、炎症因子及并发症发生情况。结果:观察组BPD严重程度低于对照组,差异有统计学意义(P<0.05);观察组辅助通气时间、需氧时间、住院时间分别为(23.58±3.15)、(38.79±4.12)、(43.85±4.52)d,均短于对照组的(28.98±3.46)、(44.82±4.49)、(50.78±5.63)d,差异均有统计学意义(P<0.05);观察组治疗后动脉血氧分压(PaO2)为(58.63±5.49)mmHg,高于对照组的(52.74±5.35)mm Hg,差异均有统计学意义(P<0.05);观察组治疗后白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平分别为(65.28±5.43)、(32.32±4.23)、(85.63±6.25)pg/mL,均低于对照组的(70.63±5.79)、(40.52±4.58)、(90.45±6.82)pg/mL,差异均有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:糖皮质激素治疗早产儿BPD可缓解病情,减轻体内炎症反应,纠正血气异常,且安全可行。
Objective:To investigate the clinical effect of glucocorticoid on preterm infant with bronchopulmonary dysplasia(BPD).Method:A total of 72 children with BPD admitted to NICU of Xinyu People's Hospital from June 2020 to June 2022 were selected and divided into two groups according to random number table method,with 36 cases in each group.The control group was treated with routine treatment,and the observation group was treated with glucocorticoid.BPD severity,clinical indexes,blood gas indexes,inflammatory factors and complications were compared between the two groups.Result:The severity of BPD in observation group was lower than that in control group,the difference was statistically significant(P<0.05).The assisted ventilation time,oxygen demand time and hospitalization time in the observation group respectively were(23.58±3.15),(38.79±4.12)and(43.85±4.52)d,which were shorter than(28.98±3.46),(44.82±4.49)and(50.78±5.63)d in the control group,the differences were statistically significant(P<0.05).After treatment,the arterial partial pressure of oxygen(PaO2)of the observation group was(58.63±5.49)mmHg,which was higher than(52.74±5.35)mmHg of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in the observation group respectively were(65.28±5.43),(32.32±4.23)and(85.63±6.25)pg/mL,which were lower than(70.63±5.79),(40.52±4.58)and(90.45±6.82)pg/mL in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Glucocorticoid in the treatment of preterm infant with BPD can alleviate the disease,reduce inflammatory reaction in the body,correct abnormal blood gas,and is safe and feasible.
