摘要
目的分析床旁超声测量视神经鞘直径(ONSD)在脓毒症相关脑病(SAE)中的临床应用价值。方法回顾性分析2022年1-12月该院收治的40例脓毒症患者的临床资料,根据是否发生SAE分为SAE组和非SAE组,各20例,比较两组一般资料和治疗前后床旁超声测量的ONSD差异,进行28 d随访,比较两组存活、死亡患者的床旁超声测量ONSD情况。结果两组一般资料比较,差异均无统计学意义(P>0.05)。两组ONSD、APACHEⅡ评分及SOFA评分不存在时间与分组的交互作用(P>0.05)。治疗前、治疗1 d、治疗3 d、治疗5 d ONSD、APACHEⅡ评分及SOFA评分均呈下降趋势(F时间=16.420、15.770、24.360,P<0.05),且SAE组的ONSD、APACHEⅡ评分及SOFA评分均高于非SAE组(F组间=10.930、19.450、28.470,P<0.05)。SAE组、非SAE组中存活患者的ONSD均低于死亡患者(P<0.05),但SAE组、非SAE组间存活患者的ONSD比较,差异无统计学意义(P>0.05)。结论床旁超声测量ONSD对SAE诊断、疗效评估以及预后判断有着重要价值,值得推广应用到临床。
Objective To analyze the clinical application value of the optic nerve sheath diameter(ONSD)measured by bedside ultrasound in sepsis-related encephalopathy(SAE).Methods The clinical data of 40 patients with sepsis admitted and treated in this hospital from January to December 2022 were retrospectively analyzed.The patients were divided into the SAE group and non-SAE group according to whether SAE occurring,20 cases in each group.The general data,ONSD difference measured by bedside ultrasound before and after treatment were compared between the two groups.The 28 d follow-up was conducted.The ONSD measurement situation was compared between the survival group and death group.Results There was no statistically significant difference in the general data between the two groups(P>0.05).There was no interaction of time and grouping in the ONSD,APACHEⅡand SOFA scores between the two groups(P>0.05).The ONSD,APACHEⅡscores and SOFA scores before treatment and on 1,3,5 d after treatment showed a decreasing trend(F time=16.420,15.770,24.360,P<0.05),moreover the ONSD,APACHEⅡand SOFA scores in the SAE group were higher than those in the non-SAE group(F inter group=10.930,19.450,28.470,P<0.05).ONSD in the patients with survive in the SAE group and non-SAE group was lower than that in the patients with death(P<0.05),but ONSD in the survival patients had no statistically significant difference between the SAE group and non-SAE group(P>0.05).Conclusion ONSD by bedside ultrasound measurement is of great value for SAE diagnosis,efficacy evaluation and prognosis judgment which is worthy of promotion and application in clinic.
