摘要
目的为了解甘肃省尘肺病患者的生存状况及其影响因素,制定科学的防治措施。方法采用Cox比例风险回归分析模型来识别和评估影响尘肺病人生存的相关因素。结果2009-2019年全省共报告尘肺病1484例,死亡267例,病死率为17.99%。尘肺期别、尘肺类型和接尘工龄是影响尘肺患者生存的危险因素。与尘肺壹期患者相比,尘肺叁期患者其死亡风险显著增高,风险比(Hazard Ratio,HR)及其95%CI为2.16(1.504~3.102);其他尘肺患者与矽肺患者相比较,其死亡风险明显下降,HR及其95%CI为0.3(0.131~0.685);随着接尘时间的增加,尘肺患者的死亡风险也逐渐增加,接尘工龄10~<20、20~<30、≥30年的HR及其95%CI分别为1.461(1.01~2.114)、1.870(1.268~2.757)、1.956(1.285~2.979)。结论应做好尘肺的三级预防,加大重点尘肺病的防治,加强职业健康监护工作,及早地筛查出疑似尘肺样变的劳动者,延缓疾病进展。
Objective In order to understand the survival status and influencing factors of patients with pneumoconiosis in Gansu Province and formulate scientific prevention and control measures.Methods The Cox proportional hazards regression analysis model was used to identify and evaluate the relevant factors influencing the survival of pneumoconiosis patients.Results From 2009 to 2019,1484 people and 267 deaths were reported in the province,with a case fatality rate of 17.99%.Pneumoconiosis stage,pneumoconiosis type,and dust exposure time were risk factors influencing the survival of pneumoconiosis.Compared with patients with stage I pneumoconiosis,the risk of death in patients with stage III pneumoconiosis was significantly increased,with the hazard ratio(HR)and its 95%CI was 2.16(1.504-3.102);Compared with patients with silicosis,the risk of death was significantly reduced in other patients with pneumoconiosis,and its HR(95%CI)were 0.3(0.131-0.685);With the increase of dust exposure time,the mortality risk of pneumoconiosis patients gradually increased,and the mortality risk rates(95%CI)of dust exposure 10~<20 years,20-<30 years,and≥30 years were 1.461(1.01-2.114),1.870(1.268-2.757)and 1.956(1.285-2.979),respectively.Conclusion Tertiary prevention of pneumoconiosis should be taken;the prevention and treatment of key pneumoconiosis should be improved;occupational health monitoring should be strengthened;and workers with suspected pneumoconiosis should be screened out as early as possible to delay the progression of the disease.
