摘要
目的:探讨综合康复策略对颈椎骨折脱位并发脊髓损伤术后患者颈椎功能及并发症的影响.方法:选取本院2019年3月至2022年6月收治的颈椎骨折脱位并发脊髓损伤患者70例作为研究对象.按照随机数表法将其分为观察组(n=35,在常规护理干预基础上采取综合康复策略进行护理干预)和对照组(n=35,采取常规护理干预).两组均进行1年以上的随访.比较两组患者术后感染发生率、护理满意率、随访期间并发症发生率,以及护理前、随访半年、随访1年时日本骨科学会评分系统(JOA)评分.结果:观察组患者术后感染发生率与随访期间并发症发生率均显著低于对照组,而护理满意率则显著高于对照组,差异均有统计学意义(P<0.05).两组患者护理前JOA评分比较,差异无统计学意义(P>0.05);随访半年、随访1年时两组JOA评分均有升高,而观察组JOA评分显著高于同时间点对照组JOA评分,差异均有统计学意义(P<0.05).结论:颈椎骨折脱位并发脊髓损伤手术患者采取综合康复策略护理干预,不仅可以减少术后感染发生,而且能提高护理满意率,降低并发症发生率,并改善JOA评分,值得借鉴.
Objective:To investigate the effect of comprehensive rehabilitation strategies on cervical spine function and complications in postoperative patients with cervical spine fracture and dislocation complicated by spinal cord injury.Methods:Seventy patients with cervical spine fractures and dislocations complicated by spinal cord injury admitted to our hospital from March 2019 to June 2022 were selected in this study.According to the random number table method,they were divided into an observation group(n=35,using comprehensive rehabilitation strategies nursing intervention)and a control group(n=35,using routine nursing intervention).They were followed up for more than 1 years.The incidence of postoperative infection,nursing satisfaction rate,incidence of complications during follow-up,and Japanese Orthopaedic Association scores(JOA)scores before nursing,half a year of follow-up,and 1 year of follow-up were compared between the two groups.Results:The incidence of postoperative infection and complications during follow-up were significantly lower in the observation group than those in the control group,and the nursing satisfaction rate was significantly higher in the observation group than that in the control group,with statistically significant difference(P<0.05).There was no statistically significant difference in JOA scores between the two groups before nursing(P>0.05).At half a year and one year of fllow-up,the JOA scores of both groups increased,and the JOA scores of the observation group were significantly higher than those of the control group at the same time point,with statistically significant differences(P<0.05).Conclusion:The comprehensive rehabilitation strategy nursing intervention for postoperative patients with cervical spine fracture and dislocation complicated by spinal cord injury can not only reduce postoperative infection,but also improve nursing satisfaction rate,reduce complications,and improve JOA score,which is worth learning from.
