摘要
目的:研究分柱显露手术与经皮内固定治疗在运动员Lisfranc损伤中的应用效果。方法:选取上海市东方医院吉安医院2017年2月—2022年2月收治的83例Lisfranc损伤运动员开展研究,以电脑随机数字表法将其分成A组(n=41)及B组(n=42)。B组选用经皮内固定手术治疗,A组选用分柱显露手术治疗。对比两组疗效、疼痛情况、术后并发症情况及生活质量。结果:A组优良率高于B组(95.12%vs.73.81%)(P<0.05)。经手术治疗后,A组及B组的视觉模拟评分法(VAS)评分均显著降低,且A组低于B组(P<0.05)。A组与B组感染、创伤性关节炎及内固定松动发生率比较,差异均无统计学意义(P>0.05)。经手术治疗后,A组及B组生活质量综合评定量表(GQOLI-74)各项评分均显著升高,且A组均高于B组(P<0.05)。结论:分柱显露手术与经皮内固定治疗在运动员Lisfranc损伤中均有一定的效果,其中分柱显露手术效果更佳,可有效减轻运动员术后疼痛程度,提高生活质量。
Objective:To study the application effect of split column exposure surgery and percutaneous internal fixation in the treatment of Lisfranc injury in athlete.Method:A total of 83 athletes with Lisfranc injury admitted to Shanghai East Hospital Ji’an Hospital from February 2017 to February 2022 were selected for this study,and they were divided into A group(n=41)and B group(n=42)according to computer random number table method.B group was treated with percutaneous internal fixation,A group was treated with split column exposure surgery.The efficacy,pain condition,postoperative complications condition and quality of life of the two groups were compared.Result:The excellent and good rate of A group was higher than that of B group(95.12%vs.73.81%)(P<0.05).After surgical treatment,the visual analogue scale(VAS)scores of A group and B group were significantly decreased,and that in A group was lower than that in B group(P<0.05).There were no significant differences in the incidences of infection,traumatic arthritis and internal fixation loosening between A group and B group(P>0.05).After surgical treatment,the scores of generic quality of life inventory-74(GQOLI-74)in A group and B group were significantly increased,and those in A group were higher than those in B group(P<0.05).Conclusion:Both split column exposure surgery and percutaneous internal fixation have certain effects in the treatment of Lisfranc injury in athlete,among which the split column exposure surgery has a better effect,which can effectively reduce the postoperative pain of athletes and improve their quality of life.
